What to expect when you call the DFA Appointment Hotline (02) 737-1000 All passport applicants scheduled

for appearance must appear physically at the DFA at the date and time of their scheduled personal appearance for data and image capturing. This applies to all passport applicants, aging from 0 years old onwards. DFA is strict about the personal appearance system, absolutely no chaperone allowed for passport applicants aging 18 years old onwards.
1. 2. 3. 4. 5. 6. Your call will be received by a Customer Care Officer who will assist you in setting an appointment for your passport application. After getting your appointment schedule, you will be asked to provide your personal information for the passport application. You will be advised on the applicable requirements you need to bring for your passport application. You will receive a reference number relative to your appointment schedule. Go to megamall.passport.com.ph/print to print your application form using the Print Code given by the Customer Care Officer. On your appointment date, proceed to the DFA Consular Office located at 7th Floor Megamall Building C, EDSA corner J. Vargas Avenue, Mandaluyong City, Philippines (see locator map). You must be at the DFA Consular Office at your exact appointment time. Early/Late comers will not be entertained. 7. 8. 9. Present your printed application form to the Appointment Counter. Get your queue number at the Information Counter and sit on the waiting section of the Processing Area. Wait for your queue number to appear on the monitor indicating what processing window you will go to. After processing, proceed to the Cashier's area. Pay the applicable passport processing fees (see passport fees). 10. After payment, proceed to the Encoding Section for data capturing. 11. After the Encoding Section, you can have your new passport released and delivered to your home without having to return to the DFA on the release date (optional). If availing of the delivery, proceed to the Delivery Counter to pay for delivery fee.

Please take note of the requirements that you need to bring to the DFA Consular Office on your appointment date. Choose an appointment DATE and TIME. Check your email for your reference number and a link to print your application form. You will need to bring the printed application form to the DFA on your appointment. Birth Certificate (BC) in Security Paper (SECPA) issued by the National Statistics Office (NSO) or Certified True Copy (CTC) of BC issued by the Local Civil Registrar (LCR) and duly authenticated by NSO. Transcribed Birth Certificate from the LCR is required when entries in NSO Birth Certificate are blurred or unreadable. (Report of Birth duly authenticated by NSO if born abroad)

No need to submit a passport size photo Valid picture IDs and supporting documents to prove identity (Please refer to List of Acceptable IDs and List of Supporting Documents)

NSO Birth Certificate Birth Certificate (BC) in Security Paper (SECPA) issued by the National Statistics Office (NSO) or Certified True Copy (CTC) of BC issued by the Local Civil Registrar duly authenticated by NSO depending on the NSO signatory.

LIST OF ACCEPTABLE IDS (At least 1 of the following):

Voters ID NSO Marriage Contract Land Title Seamans Book Elementary or High School Form 137 or Transcript of Records with readable dry seal Government Service Record NBI Clearance Police Clearance Barangay Clearance Old documents issued at least one year prior to date of application that show correct name, date and place of birth, picture and signature of applicant such as the following:

NSO Birth Certificate of child / children SSS-E1 Form or Microfilmed Copy of SSS-E1 Form Income Tax Return Voters Certification, List of Voters and Voters Registration Record (for newly issued VRR, please attach receipt) School Yearbook Community Tax Certificate
