We are pleased to submit our preliminary proposal with package price which includes hotel accommodation, food and beverage arrangement and group transportation for your review and acceptance. Prices quoted are based on a minimum group size of 20 people. Kindly note that change in traveling date, program, group size, package inclusions, meals pricing, rates of hotels and vendors, government taxes and/or currency fluctuation will require re-costing. We wish to inform you that we have not blocked any hotel rooms and function space for the group at the time of writing. Confirmation will be subject to availability at the time of reservation. In view of the large group size, we would appreciate your early acceptance and confirmation so that we can start organizing and planning for the upcoming event. Upon confirmation, a deposit of 30% of the package price is required in order to confirm the hotel room block and function space as well as other necessary travel arrangements on a definite basis. Terms and Conditions


1. Reservation A name list of all travelers (names as in passport) is required at the time of reservation. Failure to provide accurate name information can result in problems with security and control authorities and passenger processing. 2. Deposit & Full Payment Full payment is required upon confirmation of the event. Confirmation of Booking Once the signed contract and deposit(s) are received, the booking is then confirmed on a definite basis. Until such time, the booking will remain tentative. If the deposit amounts have been paid and the signed contract is still outstanding, it will be deemed that you have accepted all of the terms and conditions of our service. 4. Cancellation In the event of cancellation after settlement of deposit and / or full payment, the customer / traveler is required to submit a written notice to avoid any misunderstandings. The following cancellation charges (per person basis) will apply: After confirmation : 10 % of package price 31 60 days to departure : 30 % of package price 15 30 days to departure : 50 % of package price 07 - 14 days to departure : 75 % of package price Less than 07 days to departure : 100 % of package price Note: Hotel or any other service providers cancellation policy will take precedence in the event their cancellation charge is relatively higher than what is stipulated above. 5. Unused Services After commencement of event / travel, no refund, in part or in full, will be given for services included in the program which are partially or totally not utilized. 6. Items Not Included in the Package Items not included in the package are excess baggage charges, telephone calls, beverages, wine, laundry, tips and gratuities to porters, guide and driver and all items of a purely personal nature unless specified otherwise. 7. Force Majeure The performance of this Agreement by either party is subject to acts of God, government authority, disaster, strikes, civil disorders, or other emergencies, any of which make it illegal or impossible to provide the facilities and/or services for your meeting. It is provided that this Agreement may be terminated for any one or more of such reasons by written notice from one party to the other without liability.

8. Responsibilities Corporate Information Travel Sdn Bhd (hereinafter referred to as CIT) and/or its associated agent/s act merely as agents to other principals who supply travel services. As such, CIT and/or its associated agent/s and/or other principals shall not be held liable for any delay or diversions, change, omission or cancellation of services or any loss, damage or injury suffered by customer/traveler as a result of negligent or deliberate act/s of any person, employee, agent, servant or representative outside of its direct employment or any unforeseen occurrences beyond its control including, but not limited to war, civil disturbance, pilferage, delays, severe weather, all factors that tantamount to and deemed as Acts of God, Acts of Government, Acts of Terrorism, accidents to or failure of machinery, equipment, vehicles or industrial disputes or for any misadventure or casualty. By purchasing and accepting our services, it is deemed that the customer/traveler fully accept the conditions, terms and tariffs, which are set by the individual suppliers and/or other principals for the exchange of their services. CIT and/or its associated agent/s further reserves the right to demand, postpone, or cancel any or all parts of a program / tour arrangement under unfavorable conditions. Should a tour or an arrangement be canceled, equitable refund shall be made to each customer / traveler but the refund sum shall not exceed the original amount paid for the event / package.

SIGNATURE This contract, with exhibits attached (if any), constitutes the entire agreement between the parties and may not be amended or changed unless done so in writing signed by Pfizer (M) Sdn Bhd and Corporate Information Travel Sdn Bhd. If a fax transmittal is used by either party, then the fax copy shall serve as an original until an actual original is executed and received by both parties. The undersigned represent that they are authorized to sign and enter into this contract.
