March 04, 2013 Dear Parents The 4th and 5th year BS Electronics and Communication, Geodetic and Industrial and Management Engineering students enrolled in Environmental Engineering subject in which your son/daughter is taking this semester, will go on a local field trip on March 13, 2013. The class will be visiting selected industries to gather the following information: 1.Description of the process operation of the company/industry. 2.Identification of the waste sources, its characteristics and the treatment processes employed by the industry designed for environmental degradation prevention. After the said activity, each student will submit a detailed written report as part of their requirements for the course. In as much as the activity will be outside of the school, we are required to secure the permission of the parents/guardian concerned. It is requested therefore that you accomplish the attached PARENT/GUARDIAN WAIVER STATEMENT. Each student will shoulder his/her own expense. Pocket money is not included. The financial requirements per student are as follows: Fare (hired bus) Certificate of appreciation and tokens for Plants visited Food (Pack lunch) PhP 100.00 PhP 30.00 PhP 50.00 PhP 180.00

The following are some of the precautionary measures that will be observed during the field trip: 1. The students will be accompanied by chaperone at all times during the trip. 2. The students will be required to wear their WMSU identification card, electronics and communication engineering, geodetic engineering and industrial and management engineering T-shirt, long pants and closed shoes. 3. Roll call of all students will be made before boarding the bus after each plant visit. The undersigned, course instructors will be the chaperone for the said field trip. Truly yours,
