I. ACKNOWLEDGMENT I want to thanks all the people who helped me to get through with this On the Job Training. Of course my family and and friends whos always been there when I feel tired and give me so much inspiration. I also want to thanks my adviser whose been very supportive not only to me but to the whole class. And most especially to all the employees that I worked. They are fun to work with. They give me so much knowledge when in comes in working. And also learning how to handle different situation.

II. INTRODUCTION Being in College there are so many things that we need to accomplish before we reach the top. And one of these is the on-thejob training. On-the-job training is one of the key for our goal. We have to trained and do what our bosses do. We our being train by the company, a preparation for the future so that we already know we need and want to do/ to be. I want to show you what Im doing on Philippine Heart Center. And Human Resource Division, were I assigned. At first, I thought it was easy for me to do those things but as the day came by it getting harder and harder and kinda complicated for me to do the other task. I can say now that Ive learned a lot and grown better.

OJT PORTFOLIO IV. OBSERVATIONS IN THE COMPANY What I have observed at Philippine Heart Center are, most of the employees are very approachable and all the Nurses. And most especially the facilities are clean. The strength and challenges of this company is that, think the best practice of this company is they very specialize when it comes to heart disease. And other diseases. They also have the best nurses that is very professional. My over-all observation in Philippine Heart Center is I think they have motto that Time is Gold. They want to finish the on time or as early as the given time. And they are very aware of their surroundings. V. PERIODIC REPORTS (daily, weekly, monthly) What we do daily are the following: filing, encoding. updating loans, photocopying receiving, answering phone calls. What we do weekly are the following: receiving, answering phone calls, encoding. updating loans, photocopying, filing. What we do monthly are the following: making pay-slip, updating loans, deductions, receiving, answering phone calls, making voucher, making remittances, encoding. During my whole training in Philippine Heart Center, I think the problem/ s that I have encountered are updating loans, at first it was too hard for me to do this because is all about money, numbers. And also, by receiving, sometimes I went to a wrong division because of the unfamiliarity of the place. I think in my 300 hours of being in this company thats the biggest problem that I encountered.

OJT PORTFOLIO VII. SELF-ASSESSMENT A. Skills and current technology learned/enforced: I learned how to used the Electronic Typewriter. Its kinda difficult for me to do use it at first. And now, I know how to handle well the tasks. Time Management.

B. Equipment, tools, testing apparatus handled (softwares used included) In my over all working hours in PHC, most of the equipment that I used are computer, Xerox Machine, Logbook, and Papers. Everyday Im using these equipment to my tasks.

C. Strong points versus weak points I am at my best when it comes to encoding, receiving, filing. Although I know how to do the tasks. Sometimes Its hard for me to update the employees loans and deduction when it comes to database. Cause there are misfile and some reference that are not applicable to the employee. And also, the o.r numbers and the date when they paid for their loans. Its very complicated that I really need the supervision of my boss when doing that.

You know, these persons are the person you are not used to be with. But despite of that you break the wall just to know them and to be friends with them. Im glad that Ive met these people whos very hard working to work but knows how to have fun. They inspired me a lot. And also to my co-ojts. E. Evidence of Background preparation:
