I. ACKNOWLEDGMENT I want to give an acknowledgement for all the person who helped me to make this portfolio as the best as it could be. To my parent who give my financial support and all my needs while making. To my Big bosses who evaluated me honestly according to my performance and to all the employees who make my On-The-Job training more memorable. A special thanks to Sir. Rene for respecting all the interns and treating us as an employee too. For giving me some snack when break-time and putting happiness in our haggard day. INTRODUCTION Having an On-The-Job training on my past year level helped me a lot to coup-up to my new environment. And to be honest that experience was give me the edge to all my co-interns. The National Kidney and Transplant Institute is a tertiary medical specialty center with a significant history relevant to the medical profession in the Philippines. It is the center for referral of kidney patients from various regional hospitals in the nation and is recognized as the lead agency in voluntary blood services. With the help of this company it enhance my capability and improve my knowledge in working era. Talking with patients let me know that patience is really a virtue. And how to control my emotions and be jolly always and calm.

We shall be the major resource for the upliftment of the above medical services for government/private hospitals. We shall sustain our competitive advantage by attaining financial stability, upgrading and maintaining our facilities and providing continuing medical education to enhance the skills of our specialists and workers. MISSION To Work hand-in-hand with government in the achievement of good health for the Filipino people by providing specialized medical services, specifically in the prevention and treatment of end-stage renal diseases and other end-stage organ failure through dialysis and transplantation. To pursue excellence in developing and establishing the highest level of training and research for physicians and paramedical personnel in areas of treatment in dialysis, kidney, liver, pancreas, cell and bone marrow transplantation. To assist other government/private hospitals to develop and set-up dialysis and transplantation units, especially outside Metro Manila.

OBSERVATIONS IN THE COMPANY What I have observed at National Kidney and Transplant Institute in Medical Records are they have a strong rules and protocols, they doing their job as the best as they can, they dont want to have a back-lag on their works as possible and they implementing what is a process no special person no matter if you are rich or cannot afford to buy luxurious things as long as you are a patient you will be treated as a patient. And being calm to everyone is definitely needed. The strength of this company is they have a comfortable facilities and their environment is so clean and refreshing areas they are also preventing to use more than what they need. Challenges as I have been observed is some doctors who are not coming in time at the medical records are causing back-lag to the requests. I think the best practice of this company is theyre making their best to helped the other government/private hospital and sustaining their competitive advantage by attaining financial stability, upgrading and maintaining their facilities and providing continuing medical education to enhance the skills of their specialists and workers. Generally speaking my observation in this company is so far so good. V. PERIODIC REPORTS (daily, weekly, monthly) What we do daily are the following: Grooming every charts per box for re-scanning of the records. Receiving charts in every department and divisions. Retrieving files for the requests of the patients. What we do weekly are the following: Disposing charts from year 2006 and below Facing the patients and taking their files and request What we do monthly are the following: Time in and out During my whole training in Medical records, I think the problem/s that I have encountered is just the complain of the patient and their being demanding up to the point that they are shouting and crying in the front of me, and I dont have a choice but to say sorry.

OJT PORTFOLIO VII. SELF-ASSESSMENT A. Skills and current technology learned/enforced: Generally speaking, in my on-the-job training it improves my reading comprehensive and checking the difference in an information because it needed me a lot to check if there is a mistake. It also enhance my social leaving in facing the different person such as employees and patients. B. Equipment, tools, testing apparatus handled (softwares used included) There is no such equipment I used except on computer for checking the updates of the charts and retrieving files. They also have the program that only the employees can open. This program was also known as Medical System. With this they can tract all the records of the patient and they can also see the updates.

C. Strong points versus weak points I think the strong points in the medical records is for being professional of every staff if time to work its only for work and they are time conscious. They have a responsible Office In Charge to check every little things in the division. I think the weak point in the medical record is the conflict between the employees and forming different groups. Sometimes, being tardiness of some doctors affect the process of the request of the patients. That can cause the angriness of the claimer.

Best experiences on the job: My best experience being an intern at medical records is ofcourse is when my limited knowledge access other information that can helped me in my future. E. Evidence of Background preparation: Faults committed (its causes and suggestion given by the supervisor) Fabia is funny and enthusiastic. When it comes to work hes serious. There were sometime that he is insensitive but he is open minded and he accept criticism in a good way.
