I. ACKNOWLEDGMENT

I would like to convey my thankfulness and gratitude to you and your staff for the booming OJT which I have undergone in your progressive company. The acquaintance, skills and craftsmanship which I have educated throughout my practical training will be big help in my future. The 300 hours OJT which I have completed meets the requirement to past the PRACTICUM II. I am also thanking Mr. Ricardo C. Limpin and the entire of the Engineering and Maintenance crew for their generous support, coaching and companionship during my OJT. I have learned a lot from them. This On The-Job Training (OJT) is best practice of being a good worker someday, This OJT teach me a lot of things, being punctual, not being tardy and do all your best for the task that gave them to you. Ive learned to be more effective and efficient to the work that I do. For me to be a good employee in the future.

OJT PORTFOLIO IV. OBSERVATIONS IN THE COMPANY What I have observed at PHILIPPINE HEART CENTER is they employees are kind and friendly and responsible to their job/work. The strength and challenges of this company is that they handle their OJTs very well they trained them so much. I think the best practice of this company is they handle their OJTs well. They trained them well. My over-all observation in the ENGINEERING & MAINTENANACE DIVISION the employees are kind but they are strict to their OJTs. V. PERIODIC REPORTS (daily, weekly, monthly) What we do daily are the following: answering phone calls releasing papers Xerox What we do weekly are the following: Releasing papers Answering phone calls What we do monthly are the following: Answering phone calls During my whole training in PHILIPPINE HEART CENTER, I think the problem/s that I have encountered when I do mistake in typing but my boss said always be focused and practice typing.
