FACTS

Petitioner (hereinafter; John-Henry: Doe) hereby move

this court of public record to take mandatory judicial notice of the known facts that Judge John H Doe has knowingly, willingly, intentionally, with forethought, malice and vexation violated the YOUR STATE Trial Rules, YOUR STATE Rules of Judicial Conduct, and the law of the land which is the Constitution for the United States of America and for the State of YOUR STATE. Judge John H Doe has the appearance of bias and prejudice and

therefore shall immediately recuse himself in this matter without delay.

Petitioner hereby moves the court to take mandatory

judicial notice of the fact that by this motion to disqualify and recuse, Judge John H Doe shall immediately recuse himself without delay.

Petitioner hereby moves the court to take mandatory

judicial notice of the fact that petitioner strongly believes that Judge John H Doe has the appearance of bias and prejudice, and has created the appearance of impropriety and shall immediately recuse himself without delay.

Petitioner hereby moves the court to also take

mandatory judicial notice of the fact that Judge John H Doe has knowingly, willingly, intentionally, with forethought, malice and vexation violated the Rules of Judicial Conduct specifically Cannon 1 Rule 1.1, & 1.2, Cannon 2 Rule 2.1, 2.2, 2.3, 2.4, 2.5, 2.6, 2.11,& 2.15,. Cannon 3 Rule 3.6, 3.8, & 3.11.

Petitioner hereby moves the court to also take

judicial notice of the fact that Judge John H Doe is not eligible to preside in this matter and petitioner hereby demands

that Judge John H Doe immediately disqualify and remove himself from this cause and matter without delay. Furthermore according to YOUR STATE Code of Judicial Conduct, Judge John H Doe shall recuse himself immediately in order to ensure impartiality and fairness in this matter.
