Include its historical background, definition of terms, members of the hospice care team, roles and functions of each team member and other related concepts. 4. Some health care practitioners may be concerned that discussing the end-of-life may be too distressing for some patients and their relatives. As a nurse, how will you communicate to these patients? As a nurse, one of our duties as a caregiver is to give support to all clients in need. The way I would communicate with the patient is to be frank about the fact that death is inevitable, to make the patient understand that it is normal for people to go through this stage and that it comes to every single one of us. My approach will be sincere, calm and approachable. Here I would take on the role as a counselor wherein I would allow the patient to voice out all of their thought and emotions, I wouldnt be talking as much as I would be actively listening to what the patient says. For as much as possible I would make sure that the patient will know that he comfortable throughout the experience and that all the support will be there for the patient.

In this picture there is a patient alone in his room. As one of the interventions in providing comfort for the patient is to make the room calm and quite so that the patient can relax and alleviate some of the symptoms of the disease as well as fatigue that comes with it. Rest is one of the most important things for patients because this is the time where they regain energy lost from the day and as well as allow the body to heal itself.

In this picture there is a nurse and a family with the patient laughing. This is what a nurse should do during hospice care, to make the patient comfortable and happy by allowing visitors to visit the patient and providing moral support by means of doing all of what the field of medicine can do to aid the patient in their end of life stage.

In this photo, I see a relative looking concerned for the patient. This is a common sight in hospitals where in we see patients who are terminally ill and their loved ones never leave their side. The feeling of fear, hopelessness, and the feeling of not eing able to do anything for our love ones is a very hard thing to go through. As a nurse, my role here is to provide support to the relatives by means of keeping them up to date with the status of the patient and educating them about the condition of the patient. Another one is to keep them in high hope that it is possible for recovery by means of allowing them to express their faith.

I see an old lady happy and it seems that she is enjoying life as she knows it. Here the old lady has come to the point wherein she is just enjoying the rest of her life here on earth. Reminiscing every moment that she has gone through throughout her life. The end of life stage shouldnt be agonizing, it is the point where we should ejy the wonders of life one last time before being taken. So as a nurse, my duty is to provide to utmost comfort for the patient so that she may enjoy the remainder of her years with her loved ones without any hindrances.

In this picture it seems that they are grieving because of the death of their loved one. This is the most hardest thing to accept in our lives, the death of family. This is a common sight and everyone at some point will experience this. As a nurse, hospice care should be done. Since hospice and palliative care was not achieved due to the death of the patient, bereavement care and support for the relatives would be my next priority after performing post-mortem care. Bereavement care is providing care for the grieving patients. Helping them to accept that death is inevitable and that somehow life has to move forward. Here I would explain to them that the medical staff has done all of what they can to keep the patient alive, but due to the complication of the disease, medicine wasnt enough. AN EXPERIENCE WITH A DYING PATIENT. Her voice is hoarse. Her chest is burning. Her every wheezing breath is a struggle. She has been diagnosed with lung cancer and her condition is worsening. She is no longer able to rise from her hospital bed and she begun to contemplate death. Surely life shouldn't be as painful and agonizing as she feels? Shouldn't death be a time of peace and fulfilment? After many hard days, she had come to a decision. She asked you to administer a lethal injection so that she may die peacefully and end her suffering. As a hospice nurse, you cannot stop her from dying, but can you, in all good conscience, assist her in a peaceful death? WILL YOU? Support your answer by legal and moral principles. It might be logically assumed that since every human being has a right to lif, it should follow that one has a right to end it. However this reasoning is false and misguided. Human life is inviolable. The right to life is given to everybody, not ecause they deserve it or earn it but because it has been freely given. No one has the right to end it, even if one say it is his own, because they are not the one who created it. So as a nurse I will have to say NO in aiding the patient to die. One of many reasons is that it is not legalized here in the Philippines. People

here condemn the idea of euthanasia or mercy killing because this country is primarily a Catholic country, thus it is prolife. The view of the church is that as long as a person is living and breathing even at the dying stage, all care and procedures must be done to keep him alive until he passes away naturally. The medical profession has been accused multiple time of playing God because of euthanasia, but this is not the main goal of medicine. The main goal of the profession is to do all other alternatives to keep the patient alive and as much as possible be comfortable in their dying times. So as a nurse, we would let the patient and family know about other alternatives like hospice care to be given to the patient. Another reason that I would say no is that I have no right to do so because I am not their attending physician, I only follow what he doctors order is. I will explain to the patint and family that i cannot carry on with such a request because I am not in the position to do so, only the doctor can, therefore i will notify the doctor. If for exampleI do cary on with the mercy killing, i will be held liable for everything plus the supervising doctor because there is such a rule, captain of the ship, wherein all my liabilities will extend to the doctor handling the patient. So as much as possible I will thoroughly explain myself to the patient the reason I cant do it, if in case they persist, I will call the attention of their doctor and leave it from there.
