Michele Schaaf 2012 Evaluation Positive Work Environment: 1.

Ensures effective team functioning: Able to identify /recognize when team not functioning well emotionally in regards to change initiatives. I still struggle with turning some of the negative energy around as relates to staff and CNMs. I consistently attempt to keep the lines of communication open with rounding on CNMs/staff and weigh both their positive and negative feedback regarding my performance. I make an effort to change schedule to meet with Nurse Manager to keep informed on any night shift issues/concerns. I have improved on holding staff members accountable for interactions with patients with 1:1 discussions if I notice a PR issue or it is brought to my attention. I report back regularly with Pt representatives. 2. Manages self and staff in a manner consistent with principles of shared leadership: Attended Studer conference and met once a month for 4 months with leadership from MVH ER and MVHS ER. In conjunction with physician group/unit council started 1st initiative of peer recognition of Employee/Physician of the month which has proved to a successful form of recognition. With encouragement from Nurse Manager, implemented previous devised Beads for Deeds initiative which has promoted an increase in the use of star staff recognition on the unit. Along with unit council, I work diligently on keeping this initiative at the forefront to improve staff /peer recognition. 3. Actively support/implements change : Assisted David B. with the Patient flow coordinator change initiative/ Direct bedding initiative and continuing to improve on Door to Dr. times. I support the night shift CNMs on monitoring staff for compliance and conversations on why the change was necessary. Support of Pt experience initiatives. More frequent dialogue with staff regarding the acronyms and the critical few but more toward the end of the year. 4. Consistently displays professional conduct during interactions: One of my positive attributes is that staff find that I am consistently easy to approach and talk to. My decisions are fair and I listen to both sides of the story. I use HR when faced with a performance issue. Even when faced with a difficult/frustrating situation, I maintain my composure.

5. Maintains equal and standard criterion of diversity that aligns with the organizations diversity goals: Attends diversity dialogue discussions when schedule permits. Discussion with Manager regarding diversity issues in the department. Quality 1. Evaluates patient care: I understand and can talk through quality measures with staff. Assure quality care in conjunction with CNMs through individual chart audits when issue is identified. I need to complete more random chart audits also for this purpose. I look over the random chart audits done by staff and give feedback but only toward the end of 2012. 2. Ensures quality patient care and staff competence: I am able to competently care for all patients in the Emergency Department and talk through with leadership and staff appropriate care of the diversity of patients in the ER. 3. Actively shares knowledge, information and expertise with staff and members of the interdisciplinary team: Meet monthly and correspond frequently with all members of the Door to Balloon team including physicians, quality managers, Nurse Managers and cath lab staff. I am able to recite exclusion criteria and with assistance from Dr. Bricking / Donna Gisewite/ Dr Thorton able to exclude many cases. I met/corresponded with Donna ( new quality data personnel) weekly for a couple of months to assist her with the AMI door to balloon process. 4. Seeks out new knowledge and integrates into practice: Maintained 44 hours of CEUs. 6 of those CEUs were not in management areas. I did read Managing Change with Personal Resilience and Leadership from the Inside Out . 5. Hospital/departmental competencies met: Completed on time. 6. Participates as a team member in meeting departmental specific PI goals: Exceeds on our departmental goals worksheet. Customer 1. Behaviors with internal and external customers are consistently professional and reflect organizational standards: Collaborate nightly with team leaders, ANMs from other departments, pt representatives, ER

physicians, ER CNMs and staff. I am often successful reinforcing the expectations, missions, values of the hospital and I have worked over the last few months on becoming frequently successful in these conversations/interactions. 2. Addresses internal/external customer issues: Round on staff/CNMs and patients nightly. Need to continue to work on focused rounding. Pt flow coordinator has helped tremendously with external customer service which I assist the CNM in monitoring daily. 3. Pt satisfaction goals are achieved: Exceeded departments patient satisfaction goals. Financial Stewardship 1. Prioritizes and uses time effectively: It may be too early to tell, but I notice improved tech efficiency with the new assignments which may help with the ERs length of stay times. Continue to promote assigned staff positions to improve the efficiency of the department. ( i.e PFC, T1, 1 trauma float, push-pull and RTS nurse(s). 2. Demonstrates awareness of changed in healthcare and economic trends in practice: Educate staff in staff meetings and when questions asked regarding new initiatives/ measures. 3. Manages systems to ensure effective utilization and availability of resource: Assist CNMs with managing of staff daily including decision to decrease numbers at appropriate times and according to census.
