All bonafide students are required to know and observe the following rules, regulations, policies and requirements of the school. Ignorance thereto does not excuse any student from compliance herewith.

All students of Guindaruhan National High School are strictly obliged to follow the general rules and regulations which serve as the guidelines on the code of conduct, thus imposing, implementing and making compulsory to everyone concerned. The courses of action to take into account are as follows:

1. Students are accepted to enroll in this public learning institution to study. Any student who is engaged in activities not authorized by the school authorities like joining associations, fraternities, notorious clubs and organizations that disrupt academic work would be dealt with accordingly and would be denied further enrollment in this school.

2. Students found to have violated rule no. 1 will be immediately dropped from the roster of students. Thus, punishment, prohibition entry, etc. upon recommendation of the school head are subject for investigation and approval of schools division superintendent and regional director.

3. Students, together with advisers and subject teachers are expected to conduct themselves with dignity and deportment at all times both on and off the campus and to uphold the moral standards of this school. Students are likewise expected to wear the prescribed uniform during class days except during laundry day when the students are to dress appropriately. MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This Memorandum of Agreement is made and executed by and between:

The JLR Construction and Aggregates Inc. of Cogon, Naga, Province of Cebu, Philippines, represented in this act by its president, DR. LIZBETH REGNER, M.D. with office at Cogon, Naga, Cebu, herein referred to as the FIRST PARTY.

FRANKIE E. DELOS SANTOS, Filipino, of legal age, single, with residence and postal address in Manduang, Minglanilla, Cebu, hereinafter referred to as the SECOND PARTY.

WITNESSETH:

WHEREAS, the GUINDARUHAN NATIONAL HIGH SCHOOL is very much in need of teachers due to some causes beyond its control, i.e. increase of enrollment, lack of funds and/or non-availability of regular permanent teacher items, among others.

WHEREAS, said school thru DepEd, Division of Cebu Office, is authorized to hire teachers on any emergency basis;

WHEREAS, the tenure of the service of every contractual teacher/aide is good for one school year (June 2011 - March 2012) only which shall terminate at the end of the school year;

WHEREAS, the contract is renewable subject to the performance evaluation in which the second party should have a rating of at least Very Satisfactory;

NOW THEREFORE, for and in consideration of the above premises, the parties have hereunto agreed to the following terms and condition, viz:

1. The FIRST PARTY, as a stakeholder, is deeply concerned in the development of the total personality of the pupils/students, i.e. intellectual, physical, mental, spiritual, etc.;

2. The FIRST PARTY shall provide all-out financial support and assistance to the teacher; 3. The FIRST PARTY shall assume the obligation of paying the honorarium to the hired teacher per month amounting to Php 6,000.00; 4. The FIRST PARTY shall grant Php 2,000.00 as Christmas bonus. 5. The SECOND PARTY is a holder of Professional Regulations Commission (PRC) License No. 1044223 which will expire on 06/30/2013 ;

6. The SECOND PARTY has shown his keen interest and desire to teach the pupils/students ofGuindaruhan National High School and is willing to fill up the shortage in aforesaid school;

7. The SECOND PARTY shall handle classes in accordance with the policies, rules and regulations duly prescribed by the Department of Education (DepEd); 8. The SECOND PARTY shall observe the same rules and regulations imposed upon by the regular/permanent teachers; 9. The SECOND PARTY shall be subject to the jurisdiction of this Office in the case of the commission of any administrative offense; 10. Both PARTIES shall faithfully comply with the terms and condition set forth herein; any breach of the same shall cause the recession hereof at the instance of the aggrieved party;

11. This contract shall take effect upon the confirmation/signing of the Schools Division Superintendent; 12. Any violation of the conditions of the contract shall be settled and/or filed in the agencies/counts in the Province of Cebu. PROPERTY LEASE AGREEMENT

THIS LEASE AGREEMENT, made this ___ day of June 2011, by and between , hereinafter called the "Lessor/Landlord," and ___________________________________, hereinafter called the "Lessee/Tenant," of , Philippines.

WITNESSETH:

FIRST: The Lessor, for and in consideration of the rents, covenants, and agreements of the Lessee hereafter set out, hereby leases to the Lessee for the term of twelve (12) months, commencing the _____ day of June, 2011, and ending on the _____ day of June 2012, the La Aldea Buena Mactan property/house located at Babag 1, Timpulok, Lapulapu City, Philippines.

SECOND: The Lessees/Tenants expressly agree to pay to the Lessor for the use of said property the sum of (Php ) per month rental tendered in advance in 12 monthly post dated checks or postal money orders, the first payment due on the ___ day of June 2011, and the remaining consecutively on the _____ day of each succeeding month until and including the _____ day of June 2012. Payments are made to the Lessor without demand or further notice, said demand and notice are hereby expressly waived.

In case the check bounces due to insufficiency of funds at the time of its encashment on rental due date, the Tenant is given seven (7) days from the date on which the rental is due to settle the obligation with ten (10) percent charge of the delinquent amount (base rent). In the event said delinquent amount is not paid after seven (7) days, the Lessor will be forced to file an appropriate action against the Lessee(s) for estafa, and this would be a ground for the termination of this contract/agreement. While the action for estafa is pending in court, the Lessee(s) is given thirty (30) days from the date where the delinquent amount is due, to vacate voluntarily the house or premises.

THIRD: On or prior to the execution date of this Lease Agreement, a SECURITY DEPOSIT to be paid in cash or check of (Php ) will be placed in escrow without interest at the Lessor/Landlord's safekeeping. Such deposit will be returned to the Lessee(s) less any back payment for damages to the premises if any.

FOURTH: All rent payments exclude the use of water and electricity. The Tenant(s) shall pay the monthly bills for water and electricity. The Lessees will not keep anything in or on said property which will damage or destroy the structure of the house or any part thereof or make any improvement without the written consent from the Landlord. The use of the house is for living quarters up to persons only including children. The tenant(s) shall at all times lock and secure his/her premises. The Landlord will not be responsible for the loss of properties or personal injuries due to robbery, burglary/theft on or off the premises.

FIFTH: The Lessees will not sublease or sub rent the property rented or covered by this lease, during the term of this lease, without written permission from the Lessor. The Tenants/Lessees agree to pay all repairs or replacements of electrical lighting, water faucets and other materials related to water and electricity consumption which is/are used in his/her care and utilization.

SIXTH: The Lessor or his agent will be permitted to enter at all reasonable times to view the premises, and within thirty (30) days of the expiration of the term of this Lease, the Lessor is permitted to label the premises for rent and to show the same to other parties.

SEVENTH: The Tenant(s) hereby covenant with the Landlord to keep the house or premises in good order, to use the premises as living quarters and to surrender the peaceful and quiet possession of the same at the end of the term, in as good conditions as when received, and further, that the Tenant(s) will not do, suffer, or permit anything to be done in or about the premises which will contravene against public policy nor permit the use of the property against the laws of the land, morals, health or safety policy, and for the purpose other than those of a living quarter(s).

EIGHTH: Should this Lease be placed in the hands of an attorney or barangay captain for any settlement or any officer of the law, after such default or breach, for the enforcement of any rights herein, the Lessees agree to pay reasonable attorney's fees and damages of/to the Lessor.

NINTH: The entire contract between the parties is contained in this instrument; and this contract shall be binding upon them, their heirs, successors, personal representatives or agents, and assignees.

TENTH: The Landlord/Lessor or any of his/her agents may enter the premises without formal notice in case of fire or other emergency problems which may occur during the absence of the Tenant(s).
