Please give full address (alongwith PIN CODE) only in case of change of address. Please use separate application form for each folio. All the details on the Application form should be filled in carefully and should be in block letters. Signature : The Redemption Form should be signed by all the holders in the order of holding. In case signatures of some or all Boinanza holders do not tally with those on records, the documents under objection memo will be retained by the Registrar and only an objection memo will be sent to the Boinanza holder, in an effort to safeguard against loss of Boinanza Certificate(s) in transit. If the Boinanza holders affixes thumb impression, it should be got attested from a Bank Manager or a Notary. Provision has been made on the Application Form to have the signatures of the Boinanza holders attested from a Bank Manager or a Notary. Though this is not mandatory, investors are advised to get their signatures so attested, so as to avoid delays due to signature mismatch, etc. Please provide your Bank details in the place provided in the form in order to prevent fraudulent encashment of your Redemption Warrant. Please note that as per SEBI Guidelines no application for redemption will be processed without this information. In case of loss of original warrant, duplicate redemption warrant will be issued on request only after expirty of the validity period of the original warrant and reconciliation of paid/unpaid warrants, and after submission of necessary indemnity by the Boinanza holder(s). Instructions regarding Board Resolution/Power of Attorney When a Trust / Company /Institution etc. offers Boinanzas for redemption, the application form should be accompanied by a certified copy of the Board resolution / Power of Attorney. Procedure for change of status from minor to major. In case the first unit holder was a minor and has now attained majority, please furnish the proof of age (copy of Birth Certificate or Matriculation certificate duly attested by a Gazetted Officer/Notary and the erstwhile minors specimen signature attested by the Guardian / Banker. Procedure for Change of Name. In case of change of name due to marriage or divorce, kindly furnish the affidavit on a non-judicial stamp paper duly notarized/attested by Judicial Magistrate in that regard. INSTRUCTIONS (Please read these instructions carefully, before filling in the Application form for Redemption) 1. Please tender this redemption form duly completed / signed by all the joint holders, if any, accompanied by the Boinanza Certificate/s to our Registrars, whose address is given overleaf. Any other application will not be entertained. Care may be taken while completing the portion for filling in address to enable the Registrars to send the Redemption Warrant at the correct address. Please give full address (alongwith PIN CODE) only in case of change of address. Please use separate application form for each folio. All the details on the Application form should be filled in carefully and should be in block letters. Signature : The Redemption Form should be signed by all the holders in the order of holding. In case signatures of some or all Boinanza holders do not tally with those on records, the documents under objection memo will be retained by the Registrar and only an objection memo will be sent to the Boinanza holder, in an effort to safeguard against loss of Boinanza Certificate(s) in transit. If the Boinanza holders affixes thumb impression, it should be got attested from a Bank Manager or a Notary. Provision has been made on the Application Form to have the signatures of the Boinanza holders attested from a Bank Manager or a Notary. Though this is not mandatory, investors are advised to get their signatures so attested, so as to avoid delays due to signature mismatch, etc. Please provide your Bank details in the place provided in the form in order to prevent fraudulent encashment of your Redemption Warrant. Please note that as per SEBI Guidelines no application for redemption will be processed without this information. The given Bank Account Number, Name of the Bank and Branch will be printed on the Redemption Warrant. If the Boinanza holder has availed of any loan against the Boinanza certificate(s) with any Bank and the same has been repaid, the RAF should be accompanied by the Bankers lien cancellation letter quoting our previous lien noting to avoid any delay in redemption. In case of loss of original warrant, duplicate redemption warrant will be issued on request only after expiry of the validity period of the original warrant and reconciliation of paid/unpaid warrants, and after submission of necessary indemnity by the Boinanza holder(s). Instructions regarding Board Resolution/Power of Attorney When a Trust / Company /Institution etc. offers Boinanzas for redemption, the application form should be accompanied by a certified copy of the Board resolution / Power of Attorney. Procedure for change of status from minor to major. In case the first unit holder was a minor and has now attained majority, please furnish the proof of age (copy of Birth Certificate or Matriculation certificate duly attested by a Gazetted Officer/Notary and the erstwhile minors specimen signature attested by the Guardian / Banker. Procedure for Change of Name. In case of change of name due to marriage or divorce, kindly furnish the affidavit on a non-judicial stamp paper duly notarized/attested by Judicial Magistrate in that regard. In case the change of name is due to some other reason, kindly furnish an affidavit on a non-judicial stamp paper with a copy of the gazette wherein the change of name has been notified and a copy of the Newspaper cutting wherein the change of name has been notified and a copy of the Newspaper cutting wherein the change of name has been published. 13. Procedure for deletion of name of any of the Boinanza holders due to death. Please enclose the death certificate issued by the authority competent to issue such certificate under Registration of Births and Deaths Act, 1969 either in original or copy attested by a Gazetted Officer. If death certificate is in language other than English or Hindi, a true translation thereof either in Hindi or in English, duly attested may also be sent. The date of receipt of the application at the office of Registrars will be taken as the date of acceptance. Incase the Redemption Application Form is returned under objection, date of relodgement of Redemption Application Form at the Office of the Registrars will be taken as the date of acceptance.
