TCOT Emergency Response Plan Appendix 2 Pre Incident Plan (PIP)

Pre Incident Plan (PIP) Type of Emergency/Scenario: Emergency at Height (Rescue at Height) Location of Emergency/Scenario: Terengganu Crude Oil Terminal (TCOT) Emergency at Height (Rescue at Height) Top of Tank 441,442,443,444,445,446,447 & 448 RESPONSE MAIN OBJECTIVE(S):
1 To secure all personnel from the affected area and headcount to be executed 2 To clear all blockage to the top of the tank in order for smooth access by CEFS team to the top of the tank for personnel rescue 3 To establish communication with PMO Incident Commander for potential support

IMMEDIATE RESPONSE BY:


1. CCR Panelman

ACTION
1.1 Upon being notified by witness/work leaeder about any incident at the top of any tank, immediately notify On Scene Commander (OSC) 1.2 Notify CEFS Response 1.3 Activate the siren to inform all work site personnel 2.1 Proceed to Emergency Operation Centre (EOC) 2.2 Establish contact with Damage Control Team Commander (DCTC) 2.3 Establish contact with Muster Coordinator on headcount status 3.1 Proceed to emergency scene and establish contact with Damage Control Team Leader (DCTL) once emergency has been verified.

REMARKS
As per KIPC Emergency Management Plan, all incident/emergency at height, CEFS response will be the main responder and will be supported by OPUs ERT/DCT. In this case, CEFS shall be arrived to TCOT within 4-6 minutes after being notified by OSC.

1. Damage Control Team Leader & Members

ACTION
1.1 Mobilize to affected tank and. Secure any expose personnel away from the scene. 2.1 Notify PMO Incident Commander (PMO IC) 2.2 Follow up with DCT Commander on current situation and potential incident escalation 2.3 Notify security personnel to control traffic and prevent unauthorized entry to terminal

2ND RESPONSE
1. Damage Control Team Commander (DCTC)

ACTION
1.1 Once CEFS arrives at TCOT, assist them in set up the rescue at height equipment 1.2 Update OSC on the emergency status 2.1 Follow up with Muster Coordinator to check the headcount status and any potential missing person. If any person found missing, notify DCTC for potential search & rescue 2.2 Continue updating PMO Incident Commander. CEFS will notify OSC once the emergency beyond their control and Jabatan Bomba & Penyelamat will take charge. Prepare to handover the OSC roles to Jabatan Bomba & Penyelamat instructed by Bomba. 2.3 If incident is under control (without any interference by Bomba), declare stand down and update PMO Incident Commander on the status 1.2.1 Radio 2.1.1 Radio
