Send this checklist and all supporting documentation along with your application. Please make sure that you have included the following documents with your application. Failure to do so will result in your application being returned to you. To avoid delays, ensure that you have read our policies regarding Notarized Copies; Translated Copies; and Incomplete Applications. Notarized copy of proof of legal name change (if applicable) 5. Notarized copies of your diploma, degree or certificate (and certified translation if applicable) from your entry-level physiotherapy education
Notes: If you graduated from a physiotherapy program in Pakistan, your degree must be attested by the Higher Education Commission. If you are a new graduate and have not yet received your degree certificate you may have your school submit a Graduation Verification form (see handbook) as an interim step to get your application started. We will need the official certificate to release your assessment results.

They must be attested by the Registrar or Controller of Examinations at the university. Important Note: When you ask the school to send your documents to our office using an international courier service (e.g., Federal Express, DHL Purolator, etc.) tell the school that The Alliance will verify who the sender was. If the pick up location of the courier package does not match your school address and school location, we will not be able to verify the school is the sender of this package and we will not accept it. For example, the school can not put the documents in an envelope and seal it and then give you the sealed envelope so that you can take it to the courier company and send it to us. The school must send the package to us not you. If your school is sending documents by regular mail, please ask them to include a return address or school stamp on the external envelope. Have you completed the Canadian Physiotherapy Practice Self Assessment Readiness Tool (SART)? The Canadian Physiotherapy Practice Self-Assessment Readiness Tool is an optional step designed to help internationally-educated physiotherapists understand the Canadian physiotherapy profession. It describes what physiotherapists who work in Canada are expected to know and do. It outlines the competencies, or knowledge and skills that Canadian physiotherapists must have. The Alliance does not see any of the results of your selfassessment. It is meant only to help you self-assess your physiotherapy skills based on established Canadian physiotherapy competency guidelines. It can provide you with valuable information to help you make important decisions. The Alliance encourages you to use SART to help orient you to physiotherapy in Canada. The SART tool is available in French and English. You can find it on our website under Credentialling/Applicants/Resources.
