Name of Patient: F.G.T. Age: 72 y.o. Name of Drug Generic: TELMISARTAN Ward/Bed: 3rd flr /302 Dosage, Route, Frequency, Timing Dosage: 80 mg/tab 1 tab Mechanism of Action Blocks angiotensin II from binding to receptor sites in many tissues, including vascular smooth muscle and adrenal glands. This action inhibits the vasoconstrictive and aldosteronesecreting effects of angiotensin II, which reduces blood pressure. Attending Physician: Dr. M. Dr. L. Dr. L. Dr. T. Impression/Diagnosis: Metastatic Liver Disease, DM type 2, PTB 3, R/O Extrapulmonary TB Indication To reduce risk of MI, stroke, or death from cardiovascular causes in patients at high risk who are unable to take ACE inhibitors Adverse Reactions
Atrial fibrillation, Congestive heart failure, MI, Acute renal failure, Renal dysfunction, Anemia, Thrombocytopenia, Upper respiratory tract infection
Special Precautions Use with caution to patients whose renal function depends only to the RAAS, with dehydration and hyponatremia.
Nursing Responsibilities
Check patients blood pressure regularly. Monitor liver function test results, as appropriate, and assess for evidence of drug toxicity in patients with severe hepatic disease because theyre at increased risk for toxicity from increased drug accumulation. Instruct patient to change position slowly to minimize effects of orthostatic hypotension. Urge patient to immediately notify prescriber about diarrhea, dizziness, severe nausea, or vomiting. Advise patient to drink adequate amounts of fluid during hot weather and when exercising.
