NURSE PERCEPTION ON THE IMPLEMENTATION OF NURSING CARE FOCUSED ON SPIRITUAL NEEDS IN THE INTENSIVE CARE UNIT OF RSU PKU

Nurse Perception on the Implementation of Nursing Care Focused on Spiritual Needs in the Intensive Care Unit of PKU Muhammadiyah Bantul General Hospital Heri Sutanto Student Research Project, Department of Language Training Center Muhammadiyah University of Yogyakarta, 2009 ABSTRACT Clients who fall in the critical condition in Intensive Care Unit (ICU) room, usually experience anxiety and fear of; her/his health condition, treatment, equipment of invasive procedure and possible of physical defect or death. In this phase, the clients have to get comprehensive care, including; biological, psychological, socialogical and spiritualogical elements. Spiritual need meeting was necessary to solve problems of clients. The nurse has responsibility to give a comprehensive including nursing care spiritual needs. This research purpose was to know the nurse perception on the implementation of nursing care focused on spiritual needs in the Intensive Care Unit (ICU) of PKU Muhammadiyah Bantul General Hospital. This research used non-experimental study with descriptive design and quantitative approach. Population of this research was the nurses who work in the Intensive Care Unit (ICU) Room of PKU Muhammadiyah Bantul General Hospital. Sample of this research were the associate nurses in Intensive Care Unit (ICU) Room of PKU Muhammadiyah Bantul General Hospital. Sampling technique in this research was total sample, as many as 12 associate nurses. Data were collected by using questionnaires. The result of study shows that the nurse perception about the implementation of nursing care focused on spiritual needs in the Intensive Care Unit (ICU) based on questionnaire instrument that was given to associate nurses, generally is in the mediocre category, consisting of 5 sub variables; assessment in the value of 78,47% (good category), nursing diagnosis determination in the value of 45,83% (worse category), planning in the value of 67,36% (mediocre category), performance in the value of 75,38% (mediocre category), and evaluation in the value of 69,75% (mediocre category). Conclusion of study is, the nurse perception on the implementation of nursing care focused on spiritual needs in the Intensive Care Unit (ICU) of PKU Muhammadiyah Bantul General Hospital, on the whole is in mediocre category. The findings may be useful in increasing good quality service of nursing care. Keywords: Nursing Care Plan, spiritual, Intensive Care Unit (ICU)

A. INTRODUCTION Clients who fell in the critical condition in Intensive Care Unit (ICU) room, usually experienced anxiety and fear of; her/his health condition, treatment, equipment of invasive procedure and possible of physical defect or death. In this phase, the clients have to get comprehensive care, including; biological, psychological, social and spiritual elements. Spiritual need meeting was necessary to solve problems of clients. The nurse has responsibility to give a comprehensive service including the fullfilment of spiritual needs. Religion teaches us about the health comprehensively. Health consisted of physiological, emotional, psychological and spiritual elements. God gives human illnes as a life examination. Illnes are disturbance of phisycal function, emotional function, intelectual and social function, and also disturbance of development or spiritual function on human compared to last normal condition. Human are searching for health care provider to be free from illnes. Hospital is one of health care provider. Specially for critical condition, Intensive Care Unit is prepared for comprehensive care including: biological, psychological, socialogical and

spiritualogical. Intensive Care Unit client need to give total care to save the life. Nursing care is a systematic method done by professional nurse to give; assessment, diagnose is determination, planing, implementation and evaluation to client. This research focused on spiritual care of nursing care to client on Intensive Care Unit room.

This research paper provided information about nurse perception on the implementation of nursing care focused on Spiritual Needs in the Intensive Care Unit. The data collected were based on these research questions;

a. How was the nurse perception on assessment phase of clients spiritual need? b. How was the nurse perception on diagnose determination phase of clients spiritual c. d. e.
need? How was the nurse perception on planing phase of clients spiritual need? How was the nurse perception on implementation phase of clients spiritual need? How was the nurse perception on evaluation phase of clients spiritual need? The date were taken from the nurse on Intensive Care Unit of PKU Muhammadiyah bantul general hospital and aimed at finding nurse perception on the iimplementation of nursing care focused on spiritual needs in the Intensive Care Unit. This research data has measured that the good perception of the nurse in the implementation of nursing care focused on spiritual needs in the Intensive Care Unit can reduce level of anxiety and solve the clients problem on spiritual needs.

B. LITERATURE REVIEW Illness care by professional health worker, one of them is nursing care. Nursing care is given to client by professional nurse with consent to important aspects. Dhimas (2008) defines that Perception is a human process to change, receive, plan and suppose of information from the environment. Aziz (2004) defines that Nurse is a part of health care profession to by serve of nursing care, advocating, educating, coordinating, collaborating, consulting and researching of human as individual, family and society. Potter and Perry (2005) say that nurse perceptions are; nurse view, nurse feeling, nurse interpretation, and nurse comprehension concerning to client. Munjirin (2008) defines Nursing Care focused on Spiritual Needs is systematic method by nurse together with client to determine the spiritual need for the sake of client recovery by realization of 5 phases; examination, diagnosis is determination, planning, performance, and evaluation on spiritual aspect. Indonesian Health Knowledge

Consortium (1992) defines that client is human as; bio, psychology, social and spiritual creature and client is unity of physical and spiritual that has unique characteristics. Achsanuddin (2007) says that ICU is a separated part unit in hospital by special expert and special tools to give comprehensive care to clients who has critical condition.

C. RESEARCH METHOD This research used non-experimental study with descriptive design and quantitative approach. Population of this research was the nurses who worked in the Intensive Care Unit (ICU) Room of PKU Muhammadiyah Bantul General Hospital as the one of hospital with ISO 9001:2000 accreditations in Yogyakarta Region. Sample of this research were the associate nurses in Intensive Care Unit (ICU) room of PKU Muhammadiyah Bantul General Hospital. The samples technique in this research are the total sample, as many as 12 associate nurses. This research data were collected by using questionnaires. This research has one variable that is: the nurse perception on the implementation of nursing care focused on spiritual needs in the Intensive Care Unit (ICU). This research was on November 2008 to March 2009 in Intensive Care Unit (ICU) room of PKU Muhammadiyah Bantul general hospital. Data were analyzed by; editing, scoring, percentaging, analyzing, deciding conclusion.

D. RESULT AND DISCUSION The result evidenced that characteristic of respondents in PKU Muhammadiyah Bantul General Hospital from;

Sex variable is the most of respondents are male associate nurses with the number are 7 respondents and with percentage is 58,33%. Age variable show us that the most of respondents are less than 30 years old associate nurses with the number are 8 respondents and with percentage is 66,67% Educational background variable the most of respondents are diploma III graduated associate nurses with the number are 11 respondents and with percentage is 91,67% and just 1 sarjana of nurse. Working age variable show us that the most of respondents are work on between 1 years until 5 years associate nurses with the number are 7 respondents and with percentage is 58,33%. Based on research instrument by questionnaire of nursing care to associate nurses, nurse perception about nursing care realization on meeting spiritual needs of intensive care unit client in PKU Muhammadiyah Bantul General Hospital is on mediocre category with percentage is 73,54%. Examination phase is on good category with percentage is 78,47%. It means that nurse were already capable to execute identification about client religion, nurse in mediocre capability to inspect client praying habit, and nurse capability to inspect client comprehension about relationship between spirituality with health. Diagnosis

determination phase is on worse category with percentage is 45,83%. Based on questionnaires instrument and data analyzing nurses less had capability to formulate clients spiritual problem into nursing diagnose. Planning phase is on mediocre category with percentage is 67,36%. It means that nurses are already making a plan to solve clients spiritual problem but sometimes nurse suddenly make it. Nurse prepared spiritual condition for her/his self. Implementation phase based on Nursing Intervention Classification (NIC) is on good category with percentage is 77,65%. Nurse already try to implement nursing care on meeting spiritual needs based on NIC, such as; taking time to pray, prepared spiritual equipment, facilitate client to share about her/his fear and her/his powerless. Based on Clinebell is on mediocre category with percentage is 73,12%. Nurse

already try to implement nursing care on meeting spiritual need, such as; remembering client to be obedient to God by; taking pray. Evaluation phase is on mediocre category with percentage is 68,75%. Based on questionnaires instrument and data analyze Nurses are evaluate the implementation of spiritual care, although client response on spiritual aspect is difficult to observe.

E. CONCLUSION Generally nurse perceptions about nursing care realization on meeting spiritual needs of Intensive Care Unit client in PKU Muhammadiyah Bantul general hospital are on mediocre category. Nurse perceptions about nursing care realization on meeting spiritual needs of examination phase are on good category. Diagnosis determination phase is on worse category. Planning phase is on mediocre category. Implementation phase is on mediocre category. Evaluation phase is on mediocre category.
