CONTRACT FOR SERVICE

KNOW ALL MEN BY THESE PRESENT: This contract made and entered by and between: OFFICE OF THE CITY MAYOR, DAGUPAN CITY LOCAL GOVERNMENT with the office address at CITY HALL COMPLEX, A.B. FERNANDEZ AVENUE, DAGUPAN CITY, herein represented by HON. BELEN T. FERNANDEZ, CITY MAYOR, hereinafter referred as to the First Party; -andATTY. ROBERT PASCUA, of legal age, Filipino and with office address at City Mayors Office, Dagupan City, this city, herein after referred to as Second Party; WITNESSETH THAT: 1. The First Party is in need of the services of the Second Party who shall perform work not performed by the regular personnel of the First Party; 2. The Second Party has signified his intention, to which the First Party has accepted, to provide the service needed by the latter; 3. The Second Party hereby possesses the education, experience and skills required to perform the job as described herein; 4. The Second Party hereby attests that he is not related within the fourth degree of consanguinity or affinity to the : a) Hiring authority and/or; b) Representative of the First Party; that he has not been previously dismissed from the government services by reason of an administrative offense; Thereof, the Second Party is contracted as TECHNICAL CONSULTANT from the period of ___________________________ to _________________________ duration of the MONTHLY rate of TEN THOUSAND PESOS (Php10, 000.00) to be paid on BIMONTLY basis;. 5. That in view hereof, the Second Party; is constructed as Consultant for the period of _____________________________ to _______________________ 6. That as TECHNICAL CONSULTANT, the Second Party is expected to perform the following functions: ______________________________________________________________________ ______________________________________________________________________ ______________________________________________________________________ ______________________________________________________________________ ______________________________________________________________________ 7. That the Second Party shall perform work at a time and schedule to be agreed upon by both parties; 8. That it is understood that this contract does not create an employer-employee relationship between the First Party and Second Party, that the services here rendered hereunder are not considered and will not be accredited as government service; and that the latter is not entitled to benefits enjoyed by the regular personnel of the First Party; and 9. That the services of the Second Party can be terminated prior to the expiration of this Contract for lack of funds or when their services are no longer needed;
