Form of Business Ownership Lims Paper Bag will be a sole proprietorship form of business.

It is a type of business entity that is owned and run by one individual and in which there is no legal distinction between the owner and the business. The owner receives all profits and has unlimited responsibility for all losses and debts. Organizational Structure

Manpower Requirements Lims Paper Bag will be in need of one (1) manager. The owner will be the manager. Under the manager, there would be three (3) machine operators, three (3) packagers, one (1) driver, one (1) utility and maintenance personnel and one (2) security personnel.

Machine Operator Job Descriptions:

Manages the operating of all types of heavy machines and also works to ensure that they are well maintained

Works to adjust and operate speed, and control the load of the machines Monitors the finished products obtained from the machines to ensure the quality and quantity constraints of the company

Ensures the working status of the machines and works to provide specifications to upgrade the systems if required

Maintaining a report of the total amount of goods manufactured on a day basis

Qualifications:

Must have taken short course in electronics technology (TESDA accredited) Must be a male and at least 25 years old Must be physically fit for the job

Must have good communications skills With or without experience

Salary: Php 7, 700 monthly Packager Job Descriptions: Assembles, lines, and pads cartons and containers, using hand tools Manages and examines containers, materials, and products in order to ensure that packing specifications are met Cleans containers, materials, supplies, and/or work areas Records product, packaging, and order information on specified forms and records Transports packaged goods to the stockroom

Salary: Php 7, 480 monthly Driver Job Descriptions: In charge of delivering products to intermediaries Responsible with accurate time deliveries Drives for the company employees for work related needs Helps in loading and unloading of materials and/or products Practices diligence at all times

Qualifications: Must be a male and at least 21 years old With a Professional Drivers License With experience in handling heavy vehicles Has a good grasp of directions and locations especially in Tarlac Must be physically fit

In charge in maintaining the cleanliness and order of the office and production areas

Makes sure that all circuits are shutdown after working hours Makes sure that waste materials are properly disposed Helps in repairing, revamping and maintaining a variety of mechanical items Assists other workers when needed

Qualification: At least a high school graduate At least 21 years of age At least 54 in height Must be physically fit

Salary: Php 7, 480 monthly Security Personnel Job Descriptions: To safeguard, patrol and monitor any theft or violence in the premises where he has been assigned duly

Responsible in serving his employer by providing safety against any law, violation and determining any sort of disturbances that will incur loses to the client

The services rendered by employees are better shown appreciation and importance through fair amount of compensation in the form of salaries, wages, and benefits. Salaries and wages will depend on the position, duties and responsibilities of each employee. It will be guided by the minimum wage law. Salary schedule shall be every 15th and 30th of each month. The regular operation of the business is from Monday-Friday (9 am-5 pm). The employees shall work 8 hours a day. Employees will be given 2days day-off in a week. Employees who will work over the regular working hours shall be entitled to a 30% add-up to their salary. Hours of work would be based on the records of per time card. Payment of salaries would be given in the office of the owner. Computation of salaries and overtime pay: a. First 8 hours = hourly rate x number of hours b. After 8 hours = [hourly rate x 30% of hourly rate] x number of hours added Organizational Policies Pre-Employment Requirements The following shall be necessary for every applicant before the assumption of duties: Resume with (2) recent 2x2 pictures; Photocopy of NSO Birth Certificate; Scholastic Credentials/Transcript of Records; Certificate of Employment ( from previous employer if any ); NBI Clearance; Social Security System (SSS) Form; and Tax Identification Number (TIN) (if any).

Applicants chosen for a position are mandated to go through the following: 1. Interview; 2. Medical Examination; and 3. Orientation and training Four weeks of actual training and strict supervision from the manager to introduce the hired workers to the routines and processes in the paper bags manufacturing Appearance A neat appearance greatly impacts the way our customers view us and how our employees interact on the job. All employees are provided with company uniform, which they must wear as prescribed. Attendance and Punctuality All employees should report for their scheduled shift on time. They are expected to report to work at least fifteen (15) minutes before the time. The allowance is allotted for preparation before the actual start of work. This can also help prevent employee tardiness. Continued tardiness and excessive absenteeism will affect employee performance rating and may subject to disciplinary action and may even lead to dismissal in extreme. Absences Saturdays and Sundays are official day-off of all employees. If the need to work on Saturday and Sunday arises, the employees shall be receiving reasonable additional pay and benefits/incentives. Employees are also given five days paid leaves annually. To make use of these, the employee who wishes to excuse himself from work must inform his manager immediately for the latter to make necessary adjustments. Time Card A bandy clock will be located at the entrance, near the security guard. The security guard shall monitor the punching in of the employees time card and inspect their baggage before entering to make sure that they do not bring unnecessary things inside the workplace.

Access and Inspection of the Equipment The manager reserves the right to the access, and inspection of everything that belongs to the business tools, equipment, and furnishing including desks, cabinets and files. As a consequence, no employee shall have anticipation of privacy with respect to any information or material stored in the business-owned assets. Off-duty Employees The security guard is required to inspect the employees baggage as they leave the premises of the factory. No employee shall be found taking any company property out of the plant. Unless the company management requires overtime, employees are not allowed to stay inside the grounds of the industrial unit after the normal working hours. Code of Conduct Every employee must advocate professional conduct, good sense of responsibility, and willingness to abide by the rules and regulations of the company. Failure to act in accordance with these rules and regulations will cause employment warnings and penalties. Offenses and Penalties Minor Offenses Under minor offenses, the following are the sanctions to be taken by the company equivalent to the number of times the employee incurred the established minor offenses. 1st offense: 2nd offense: 3rd offense: 4th offense: verbal warning written warning three (3) working days suspension with no pay termination

Minor offenses to be considered are the following: 1. Habitual tardiness (which includes lunch break and between periods)

2. Absence without leave 3. Not wearing the prescribed uniform 4. Smoking while on duty 5. Reporting to work under the influence of alcohol or drugs Major Offenses The sanctions for the commission of these are: 1st offense: suspension; disciplinary probation (DP) 2nd offense: final warning; strict disciplinary probation (SDP) 3rd offense: exclusion/expulsion The major offenses to be considered are the following: 1. Leaving the factory/store without a departure permit. 2. Forging or tampering with signatures, letters, permission slips, and other company documents. 3. Publishing or circulating false or malicious information about the company, an official or any employee. 4. Threatening, intimidating, provoking or coercing any member of the company community. 5. Unauthorized use of company's and company official's name for any purpose, such as to solicit funds or donations or hold unauthorized parties, balls, dances, and other activities. 6. Acts that cause physical and moral harm and injury to another. 7. Gambling in any form inside the company premises. 8. Material theft of company properties. 9. Repeated violation of major/minor offenses. 10. Improper scandalous behavior

The following rules and regulations are to be observed by the employees: All employees are required to wear the prescribed uniform while at work. No employee is allowed to bring sharp objects, deadly weapons, combustible materials, illegal drugs, and alcohol inside the workplace. Smoking shall be prohibited inside the factory. Consuming or using alcohol or illegal drugs in the workplace is also prohibited. Employees will not be allowed to work or to continue their shift if their ability to work is impaired. The work area must be kept clean and tidy all the times. All accidents or injuries, no matter how minor, must immediately be reported to the management so that corrective actions or First Aid may be taken. Employees should report hazardous workplace conditions to the management immediately. No horseplay among employees will be tolerated while on the job. The management should administer fire and earthquake safety drills at least once a year to prepare employees during a real fire. Every employee should also learn how to properly use the fire extinguisher. Any employee leaving the workplace for any reason must notify the manager of his intentions. Legal Requirements Lims Paper Bag is a newly established business organization structured as a sole proprietorship. The sole proprietorship is the simplest business form under which one can operate a business and is personally responsible for its debts. A sole proprietor need only register his or her name and secure local licenses and the sole proprietor is ready for business. Before beginning its actual operation, the owner must first comply with the rules and regulations. In order for a business to become legal, such compliance is necessary. The business will avail and enjoy variety of privileges and incentives set by the national government. It is essential to register your business to avoid any legal problems once business operations begin.

Lims Paper Bag is required to require business permits such as Certificate of Registration in Bureau of Internal Revenue, Barangay Clearance, SSS clearance Sanitary Permit to operate, Community Tax Certificate, Location/Zoning Clearance, Building/Occupancy Permit, Lease Contract, Fire Safety Clearance, Employee Health Clearance, and Certificate of the Statement of Capital Investment, Location Sketch and Mayors Permit. In accordance with the requirement of the Department of Environment and Natural Resources in order to acquire a Certificate of Registration and License to operate from DENR Region III in Camiling. It is for satisfactorily complying with the requirements to operate as a paper bag manufacturing business.
