REPUBLIC OF THE PHILIPPINES ) PROVINCE OF LA UNION ) S.S CITY OF SAN FERNANDO ) AFFIDAVIT I, Marilou C.

Cagujas, 35 years of age, married a resident of San Benito Norte, Aringay, La Union, after being sworn to an oath in accordance with law, do hereby depose and state that: 1. Janeth Farinas Plandez is a tenant of a boarding house located at Lubiano Road, Barangay Lingsat, San Fernando, La Union owned by Leopoldo Garcia to whom my sister, Marife C. Dacanay is the caretaker. 2. Plandez posed herself to us as an agent of two different recruitment agencies by showing supporting documents of her capacity to recruit and deploy persons abroad. She also mentioned to us during our conversations with her, she was actually talking to the persons whom she able to deploy abroad and already having good jobs abroad. Relying to her established reputation and I, who only reached second year high school education and have no enough knowledge with regard to recruitment, she had successfully gained my trust.

3. On March 2013, I came to Plandez to ask for help so that I can employ work abroad. She said I can only work as House Keeper in Guam. She assured to me that in two months I can be deployed abroad provided that I can pay the necessary fees the soonest possible time. She even guaranteed to me that she will be the one who will take charge of the processing of documents I needed for my application. 4. Plandez demanded the amount of P 6,500.00 for the processing of my passport and medical examination. Thereafter, on April 2013, I and Plandez went to Manila for my Medical examination and to avail my passport. 5. When we reached Manila, I have undergone my medical examinations but I was not able to avail my passport. Plandez left me in her boarding house in Manila while she went home to La Union. 6. During my stay in Manila, I was not able to talk with my sister freely. Every time I talk to her there is a person who watched over me to check if I will mention anything to my sister with regard to my application. 7. Without my knowledge, Plandez solicited money from Marife C. Dacanay, my sister, the amount of P 14,000.00 for my training and Visa going to Guam. 8. When I was already able to talk to my sister she told me that Plandez let her hear the conversation of Plandez and a man over the phone at one dawn during my stay in Manila stating that my medical examination results went good and I am fit for work. Moreover, the man also told that he needed money

to finalize my papers I needed. After successfully obtaining money from my sister, Plandez went back to Manila to pay for my VISA and for my training. 9. I was able to sign my VISA at Whocking Restaurant. However, there was no training happened. Later, I went back home to La Union. Subsequently, Ms. Plandez again demanded the amount of P 5,000.00 from me in order to facilitate the processing of my papers. Plandez did not issue any receipt of every transaction we have made. 10. Plandez promised me that I can be deployed abroad as soon as early as two months from such date. However, when the time has arrived, I was not able to go abroad. From then on, we went again to Manila for my appearance to avail my passport, to attend my training and to take necessary examinations. When we reached Manila, we did not do anything. 11. It came to my knowledge that my other co-applicants were also there to follow-up their applications. When I came to realize all of the money my family had spent, I again talked to Plandez if I can change my application from Guam to Kuwait because it is easier to go there as what she had let me witnessed when she brought me to the airport to see a proof that she can successfully deploy persons abroad. 12. Plandez agreed to what I asked for but I needed to give another amount of P 5000.00 for another set of Medical examination for my application to Kuwait. Thereby, I went for series of medical examinations. Later, we went to two different recruitment agencies and there Ms. Plandez introduced me to her niece in order to smoothen the progress of my application. 13. Series of problems arouse. I was not able to receive any of the documents Plandez promised to process for my application to wit included my NSO birth certificate which is necessary for me to avail my passport. Later, to administer my passport application, Plandez volunteered to handle my passport application so I gave her an authorization letter to enable her to avail my NSO birth certificate and be able to see to it that my birth certificate is already corrected. I even handed her my Voters Identification. Until now, there was no accomplishment of anything what she promised me. I tried to contact her but to no avail I cannot reach her to any of the contact numbers she had given me for me to retrieve full amount I have given her. 14. I am executing this affidavit to attest to the truthfulness of the allegations mentioned here and for legal intents and purposes that it may be worth.
