Scope
The purpose of this document is to establish the guidelines for preparation of Hazard & Operability (HAZOP) e!ie"#

2. Input Document
$ollo"ing input documents are re%uired during HAZOP re!ie"& (a) HAZOP re!ie" information pac'age ((nfo Pac') (b) Appro!ed for )esign* Piping & (nstrument )iagrams (P&()s) (A+ size dra"ings) (c) Appro!ed for )esign* Plot Plans (A+ size dra"ings) (d) P,$, if a!ailable at the time of HAZOP (for P)O pro-ects only) (A+ size dra"ings) (e) ,oft files (preferably #pdf files) of the dra"ings . documents for HAZOP re!ie" for display on a pro-ection screen at the HAZOP re!ie" !enue#

3. HAZOP Review Info Pack


The HAZOP re!ie" (nfo Pac' is basically a compilation of all the information needed to conduct a HAZOP re!ie" and pro!ided at least a fe" days before hand to each of the participants of the HAZOP re!ie"# The follo"ing is the content of the HAZOP re!ie" (nfo Pac'& The "ord document part of the HAZOP re!ie" (nfo Pac' contains the follo"ing information se%uentially& ,ummary & This gi!es a brief summary of the pro-ect being HAZOPed Abbre!iations and )efinitions& The abbre!iations and definitions used in the "ord document# (ntroduction& This section pro!ides the purpose of the document and the pro-ect bac'ground or brief o!er!ie" of the pro-ect under consideration# Process )escription& This section pro!ides the narrati!e for the process including the unit operations in!ol!ed and their se%uence#

,cope of HAZOP& This section lists out the dra"ings . documents that should be used for HAZOP re!ie" in a tabular form# /ode )etails& This section should pro!ide a narrati!e of the P&() 0/O)1,2 to be used during HAZOP re!ie"# These nodes shall also be reflected as high3lighted portions in the P&()s to be used for the HAZOP and part of the HAZOP re!ie" (nfo Pac'# Appendices may be pro!ided listing further documents . dra"ings for reference for the HAZOP re!ie"# This is optional and on a case3to3

case basis# $or ease of distribution of the (nfo Pac'4 A5 size dra"ings of P&()s4 plot plans and any other dra"ings listed in 0,cope of HAZOP2 should be attached to the main "ord document# The compiled (nfo Pac' including the "ord document and the dra"ing attachments shall be made into a number of copies depending on the number of participants of the HAZOP re!ie" and then distributed to the indi!idual participants# The preparation of HAZOP re!ie" (nfo Pac' copies and their distribution shall be the responsibility of the pro-ect engineer for the pro-ect to be HAZOPed#

4. HAZOP Review
4.1 Objective an! Scope (n a !ery broad sense4 HAZOP re!ie" is meant to identify any actions re%uired to alle!iate or remo!e any potential hazards or operability problems for the sub-ect plant . unit# Proper recording and reporting of the HAZOP re!ie" discussion is considered an integral part of the ob-ecti!e of HAZOP re!ie"# The scope of the HAZOP re!ie" shall be clearly stated in the HAZOP re!ie" (nfo Pac' document# As a guideline4 items such as positioning of safety sho"ers4 !al!e accessibility4 handling of process and "aste chemicals etc# are not included in a HAZOP re!ie"# 4.2 "eam #ompo ition (n order to carry out an effecti!e HAZOP it is necessary to ha!e a balanced multidiscipline team# T"o types of capability are re%uired to ha!e an effecti!e HAZOP re!ie" discussion& )etailed technical 'no"ledge of the process e#g# chemistry of the process4 process unit operations4 process control and automation etc# 6no"ledge and e7perience of applying the highly structured4 systematic HAZOP techni%ues#

The Team 8eader (also sometimes 'no"n as 9hairperson . $acilitator) of the HAZOP team should be selected for his . her ability to effecti!ely lead the HAZOP re!ie" and ha!e sufficient seniority to gi!e the re!ie" recommendations the proper le!el of authority# (deally the Team 8eader should be someone independent of the pro-ect# $or proper recording of the HAZOP re!ie" discussion points a HAZOP secretary should be part of the HAZOP re!ie" team# The HAZOP secretary should be technically %ualified to understand the HAZOP re!ie" discussion and also familiar "ith the HAZOP techni%ue# ,ecretary should be preferably from Process )iscipline#

Other team members for the HAZOP re!ie" should be selected on the basis of the positi!e contribution they can ma'e to the re!ie" through their specialist 'no"ledge and ability# The participation of an Operations . 9ommissioning specialist for a similar plant . unit is strongly recommended# A typical team composition for a HAZOP re!ie" can be as follo"s& $u%%&time o' #o'e "eam& Team 8eader ,ecretary 8ead Process 1ngineer (preferably from both 9ontractor . 9onsultant and 9lient side) 9ontrol & Automation 1ngineer (preferably from both 9ontractor . 9onsultant and 9lient side) Operations . 9ommissioning Team (includes Process and 9&A engineer) Pro-ect 1ngineer (preferably from both 9ontractor . 9onsultant and 9lient side) Pa't&time "eam Piping 1ngineer (contractor . consultant) :echanical (,tatic . otary) 1ngineer (contractor . consultant) 1lectrical 1ngineer (contractor . consultant) 9i!il 1ngineer (contractor . consultant) 4.3 P'epa'ation The HAZOP re!ie" (nfo Pac' is part of the preparation e7ercise prior to the actual HAZOP re!ie" discussion# Additionally it is the duty of the nominated participants for the HAZOP re!ie" to study the HAZOP (nfopac' in detail prior to appearing for the meeting# The Team 8eader at his . her discretion may suggest changes in the HAZOP re!ie" (nfo Pac' for better understanding of the scope of the HAZOP re!ie"# Part of the preparation in!ol!es organizing the !enue4 date4 start3time and duration of the HAZOP re!ie"# This should consider ready and continuous a!ailability of the nominated full3time members for the re!ie" and a!ailability of part3time members as per a designated time schedule# This shall be the responsibility of the concerned pro-ect engineer# The !enue (meeting room) for the HAZOP should be spacious and "ell !entilated for all the participants to participate for long hours at a stretch# :eeting schedule should include planned periodic brea's "ith refreshments because of the intense nature of the HAZOP re!ie" discussions "hich re%uires tremendous po"ers of concentration to be e7ercised# $ollo"ing accessories should be pro!ided at the HAZOP re!ie" meeting room

for smooth conductance of the HAZOP re!ie"& ;all pro-ection e%uipment hoo'ed to a computer# The pro-ection e%uipment should be able to display the dra"ings . documents to be HAZOped on the "all# The HAZOP re!ie" "or'sheet should be a!ailable as an editable document and be displayed on the pro-ection "all "hile editing# :ar'ers . High38ighters for mar'3up on dra"ings Pins. tape for stic'ing A+ size dra"ings to the "all 8aser 8ight for pointing to a certain section of the document being pro-ected on the "all#

4.4 (et)o!o%o*+ The entire process plant .unit is sub3di!ided into manageable sections for ease of understanding "hich are called nodes*# /ode* mar'3up on P&()s and its description should preferably be done during the preparation stage of the HAZOP re!ie" and be part of the HAZOP re!ie" (nfo Pac' to sa!e time during the HAZOP re!ie" discussion# The ne7t step in!ol!es using a fi7ed set of terms called guide "ords* for each process parameter (e#g# flo"4 temperature4 pressure etc#) in the selected node to identify the potential hazard or operability problem that could occur# The combination of the guide "ord and the process parameter pro!ides a de!iation in the process# $or e7ample4 the guide "ord no* combined "ith the process parameter flo"* results in the de!iation no flo"*# The follo"ing steps are recommended for conducting the HAZOP re!ie" in a systematic manner& 8ist do"n the node number in the HAZOP "or'sheet and pro!ide the follo"ing details for that node in the HAZOP "or'sheet ( efer Appendi7 A for typical format of HAZOP "or'sheet)& Pro!ide a brief description of the node in the HAZOP "or'sheet Pro!ide process data such as operating pressure4 operating temperature4 design pressure4 design temperature for the node Pro!ide the P&() tag number co!ering the selected node# (f more than one P&() is re%uired to co!er the node4 pro!ide the tag number of all P&()s# 1nter the first parameter in the column for parameter# (t is recommended to start "ith $lo"*# 1nter the guide "ord* against the parameter in the ne7t column# (t is recommended to start "ith /o*# The de!iation is /o $lo"* (dentify one cause for /o flo"* based on the process as depicted in the P&() Outline the conse%uences (both upstream and do"nstream) (dentify the protection or safeguards a!ailable

Pro!ide the recommendations . actions to eliminate . mitigate the de!iation (dentify a second cause for /o $lo"* and repeat the steps after the cause identification as abo!e until the team agrees that no more reasonable causes of /o $lo"* can be identified epeat the entire procedure until all the de!iations associated "ith the identified node ha!e been considered# After completion of an identified and numbered node mo!e to another node and repeat the entire procedure as described abo!e# The team members "ill apply their 'no"ledge and e7perience to each de!iation to identify possible causes ("ithin the scope of the HAZOP study) to establish the credibility of the e!ent# The associated conse%uences "ill be considered to determine "hether they ma'e a significant impact on the Hazards and Operability of the plant . unit# <sually there "ill be more than one cause for each de!iation and the conse%uences may !ary bet"een causes#

Typically these "ill be produced after the study in the form of an action list as "ell as indi!idual action response form for each of the action points noted in the HAZOP re!ie" meeting# $or each action item4 the responsible indi!iduals and re%uired completion dates are identified# This list "ill then be e7pedited for progress and follo"ed up till the action items ha!e been implemented and HAZOP close out report has been issued#
