INTRODUCTION: The security survey aims to identify the buildings exposure to various threats, risks, and vulnerabilities, determine which assets to secure, and recommend ways to reduce risks and vulnerabilities to an acceptable level in order to help the Management and Administration of A ! The scope of the survey covers the traditional functional areas of security which are the "hysical, "ersonnel, and nformation (ecurity. n addition, it will also try to look into some risk and ha/ards involved in safety because providing a safe environment is also a ma0or concern of security. 1. The vicinity is secured by a 95foot solid view concrete perimeter fence with some portions imposing full view concertina wire with standard 25foot barbed wire guard top guard angling at 3: degrees as the first line of defense. There were gates made of reinforced metal for the intruder to cross before reaching the interior part of the building.

3. PROTECTIVE LIGHTING a. S!'+a'!$( a" "+%,e-e. There noted busted bulbs within the building that need to be replaced. The duty personnel at night shall conduct inventory and submit results of inspection in order to rectify the not functioning lights.

4. ALARM AND COMMUNICATION SYSTEM a. S!'+a'!$( a" "+%,e-e. The hospital was installed with thirty two (87) units of ,,T; = (urveillance (ystem installed at the strategic location of the hospital with a digital video recorder and ongoing installation of sprinkler system, and other fire fighting e>uipments like 74 lbs dry chemical fire extinguishers.

Ve9!&1e" C$('%$1 : The official company drivers are authori/ed to drive any company owned vehicles provided they present a duly signed gate pass - trip ticket and their respective itinerary. .The security participation in vehicle control is to log the time of arrival and departure of each vehicle to include its passenger. Ma'e%!a1" C$('%$1: The company shall designate a personnel to receive and check all incoming deliveries or materials pull5out. The guard will check the invoice - delivery receipt and the item to be delivered or pulled5out. Any outgoing deliveries of any e>uipment or supplies must be covered by a gate pass duly signed by authori/ed representatives or department heads. FINAL SURVEY ANALYSIS : !eference the assessed deficiencies and noted weaknesses as enumerated herein, the possible advent of an untoward situation can be best pre5empted and the possible losses to lives and properties can be effectively mitigated by upgrading the whole security set5up system in accordance with the foregoing suggestions.
