1. HOUSING: HomestayinLA offers home stays with local hosts. This includes a private, furnished room. The home stay can include two (2) meals per day (breakfast and dinner); one (1) meal per day (breakfast); or no meals, according to your preference. Cost will vary according to the plan you choose. 2. APPLICATION AND PAYMENT: Please complete the application form and return to HomestayinLA by E-mail or fax. Do not send payments. HomestayinLA will invoice you. To get prices, click here. All fees are payable in US dollars only. 3. CONFIRMATION OF HOUSING: If the Application Form, non-refundable administrative fee & security deposit are received on or before 45 days prior to arrival, you will receive confirmation of your placement approximately 2-3 weeks before you are scheduled to arrive. All payments for the entire length of the contract have to be received by HomestayinLA, before the arrival day. HomestayinLA reserves the right to refuse service to any student whose complete paperwork and payments (as stated above) are not received at least 14 days prior to students arrival date. Any such situation will be handled on an individual basis... 4. ARRIVAL INFORMATION: Agents or students will inform HomestayinLA as soon as it is available as to the arrival date, flight number, airline name, and departure/arrival time. If there are any changes to the arrival information, HomestayinLA should be notified as soon as possible as new housing arrangements may need to be made. 5. HOUSING CANCELLATION: In order to receive a refund of the housing fee, (application fee and wiring costs are non-refundable), cancellations must be in writing and received by HomestayinLA, 45 days prior to your arrival. If your written cancellation is not received 45 days prior to your arrival in the U.S., you will forfeit any prepaid fees. 6. EXTENSIONS: Extensions of stay will be calculated at the prevailing market rates, which could be different from your original rate. Under no circumstances, may the student or their home stay host contract directly with one another. If the student does not wish to extend their stay, it is understood that the student is to leave their home stay at the end of the term of the housing contract.7. TERMINATION and REFUND Termination refers to any cancellations made once you have arrived and begun your housing contract. HomestayinLA reserves the right to terminate without a refund of the students home stay arrangement, if the student is found to have behaved with gross misconduct. Gross misconduct includes but is not limited to theft, abuse of property, verbal or physical abuse of persons, alcohol or drug abuse, failure to adhere to federal, state or local laws, or school regulations. In addition HomestayinLA shall inform the students advisor. No refunds whatsoever will be paid to students who have been found to have behaved with gross misconduct. No refunds are offered if students cancel the contract. Student will be unable to claim the deposit, if student stays at host house without going through HomestayinLA agency. 8. REQUEST TO CHANGE HOST: Students may request a change of homestay, with a valid reason (determined on a student by student basis), subject to availability of another home stay. HomestayinLA will attempt to resolve any issues between the student and the host prior to moving students. A minimum of a 30 day notice will apply. One move is free, if others are requested, fees apply. Home stay housing fees are not refundable. 9. TRANSPORTATION: It is the responsibility of the participant to provide his/her own transportation throughout the duration of their stay. 10. INSURANCE: HomestayinLA and its representatives are not liable for any damages, injuries, or other events that occur as a result of your U.S. stay. Obtaining proper insurance health, medical, and accident insurance is the sole responsibility of the student.

If you check off no for smoking and it is discovered that you are a smoker, you may have to be transferred to another host family if available (or hotel at students expense) you will have to pay a moving fee of $50.00. Please initial here ______________ 2. If you are choosing the breakfast only option; It is understood that the host is responsible for providing breakfast only. The host is not required to serve breakfast, but will have the food items available for you. You are responsible for purchasing all other food items such as your lunch and snack

foods, sodas, etc. Dinner is consumed outside the family home. The student does not have full access to food items in the refrigerator or the kitchen cupboards without the permission of the host. Please initial here _______________ 3. If you are choosing the room only option, I understand that the host is not responsible for providing any kind of food. You are responsible for purchasing all food and drink items etc. You may ask your host for space availability in the refrigerator and or shelf space. Dinner is consumed outside the family home. The student does not have full access to food items in the refrigerator or the kitchen cupboards without the permission of the host. 4. Making any direct agreement with a home stay host is a breach of this contract. Please refer to item #6 in the General Information and Conditions portion of the application. If the student and the host contract between themselves for the student to extend, the student will forfeit their entire security deposit. 5. It is understood that home stay and airport pick up services (if applicable) are for the student only. If your parent(s) are accompanying you to Los Angeles, they are responsible for their own accommodations. I am the parent/legal guardian of the above minor applicant. I have read, understood and accepted all the information on the homestayinLA. Application/Contract for Home-Stay and release homestayinLA from all legal responsibilities for the student above.

AGREEMENT AND RELEASE: Liability Release: Although HomestayinLA makes every effort to meet your preferences; HomestayinLA does not guarantee that your specific placement preferences will be met. I agree that I will not hold liable HomestayinLA or any individual associated with or employed by said entity in case of personal injury or property damage that may occur as a result home-stay placement. I certify that the above information is true and correct. I, the undersigned have read, understand and accept all terms and conditions of this Student Application Form Contract. I fully accept the terms of the responsibility, refunds, extensions and charges as stated above in sections 1 through 9 of this form. I also understand that any loss, injury, damage, accident, delay or expense resulting from events beyond our control, including (without limitation) acts of God, wars, strikes, incidents of politically motivated violence, sickness or quarantine, government restrictions or regulations, or in the absence of its own negligence, arising from the use of any vehicle from HomestayinLA or selection of or from any act or omission by any home stay host, bus company, landlord, taxi service, hotel, restaurant, school or other firm, agency, company or individual. I understand that I am responsible for exercising due caution and common sense at all times. I hereby indemnify HomestayinLA, its agents and employees, and hold them harmless for any financial liability or obligation which I may personally incur, or injury or damage to the person or property of others that I may cause. HomestayinLA reserves the right to make changes in my housing arrangements. I acknowledge acceptance of the terms of the refund policy as stated in the General Information and Conditions I authorize HomestayinLA to disburse monies paid by me to HomestayinLA for use on my behalf to my home stay or for housing and other services as periodically needed. I understand and agree with the extension procedure as stated in Item 6. This agreement becomes effective upon execution of this document and shall be governed by the laws of the State of California.
