Principals must check yes in the box following each question for the school to be considered eligible for textbook aid, nonpublic technology, and nursing and federal grant programs. When completed, each Principal must fax, to my attention, your ID #, your password, a copy of the completed form in its entirety, and proof of submission of the form. Fax to Dr. Marianne Conway at 973-779-0083.

E-RATE
The 2014-2015 E-Rate Application Filing window dates have been established. The filing window OPENS at 12 noon on Thursday, January 9, 2014, and CLOSES at 11:59pm on Wednesday, March 26, 2014. To comply with program rules, a school must file its Form 471 while the filing window is open, BUT a school must post a Form 470 and wait a minimum of 28 days before it files the Form 471. Note that if a school has already posted the Form 470, and the 28 day waiting period has already passed, the school still must wait until the window opens on January 9th to file its Form 471. If the school has NOT yet posted its Form 470, it still has time to post it. The school does not need to wait until January 9th; the Form 470 can be filed now. February 26, 2014, is the absolute LAST day that a school can post a Form 470 and still be able to file the Form 471 before the window closes. The absolute deadline for submitting the Form 471 for 2014-2015 is Wednesday, March 26, 2014. Item 21 attachments to the Form 471 must be submitted by the close of the filing window. Remember that all the required forms can be filed online or using paper forms. If a school chooses to submit the paper forms, allow extra time to accommodate mailing delays.
