Medication Assistance Program Annual Recertification Training


Problem Identification
The Home Care Program hires Health Care Aides to provide health care services, including medication assistance, to help maintain individuals in their own homes and out of more expensive health care institutions. Managers within the home care program are requesting additional training for Health Care Aides in medication assistance due to the large number of reports of medication errors being documented within the organization's online incident reporting system. Ongoing medication errors have the potential to affect client health negatively and create unnecessary additional costs to the health care system as a whole.

Delivery Options
The instruction for this project will be delivered as blended training that will include an online module as well as face-to-face review by health care aide supervisors at local sites. The online module will provide for review of basic information required for all medication assistance procedures as well as analysis of case studies to be followed up by face-to-face interaction with the health care aide supervisor to discuss case study answers. The online module will provide opportunities for the learner to engage with the materials through real life scenarios as well as questions to test his or her understanding of the medication assistance procedure. The face-to-face interaction with the manager will give the learners the opportunity to ask for clarification if needed and to receive performance feedback.

Terminal Objective 1: After completing the online unit of instruction, the learner will be able to identify steps included in all medication assistance procedures and how they relate to safe practice. Enabling Objectives Recall the steps included in the basic medication assistance procedure Assessment Idea Quiz: Sequence-type questions asking learners to place steps of medication assistance procedure in correct order. Absorb Activity Presentation: Short slide show to review medication assistance procedure. Reading: Provide link to Intranet for organizations medication assistance procedure (MAP) manual. Presentation: Short slide show to teach concepts of why certain steps in the medication assistance procedure are necessary to prevent medication errors. Do Activity Connect Activity Drag and drop list of steps included in the medication assistance procedure using headings: introduction, 1st safety check, 2nd safety check, 3rd safety check and follow-up care. Reflect on each step of the medication assistance procedure individually to identify where medication errors can occur. Pick multiple steps within the medication assistance procedure that could result in a medication error if not performed correctly. Rationale will be provided for correct responses. Quiz can be repeated until all correct responses are discovered. Online instruction Face-to-face

Quiz: Pick-multiple questions asking learners to select all steps within the medication assistance procedure that could result in a medication error if not performed correctly. Correct responses will provide rationale. Learner can repeat quiz as many times as needed to obtain 100%.

Terminal Objective 2: Given case studies based on real-life scenarios depicting medication errors during medication assistance, the learner will distinguish the root cause for the incident with 100% accuracy. Enabling Objectives Recall previous medication errors that have occurred Apply knowledge of root causes and past medication error experiences Assessment Idea Learners will be asked to write down examples of medication errors they have made in the past. Case Studies: Learner will formulate and write answers for each case study then discuss answers with manager for confirmation and/or clarification. Rubric will be used to assess learner for accurate identification of root causes. Absorb Activity Reading: Provide online document listing a summary of medication error reports by health care aides as noted from the incident reporting system. Story-telling: Provide tragedy story about how health care workers leaving discarded medicated skin patches in an open garbage caused the death of a 2 year old boy. Reading: Provide link to web page for further reading about this and other stories that relate to errors with medicated skin patches. Terminal Objective 3: Following completion of the annual recertification training in medication assistance, the learner will value the role and responsibility he or she has in ensuring safe and competent medication assistance as indicated by an absence of medication errors over the coming year. Enabling Objectives Examine personal beliefs and attitudes regarding factors that may contribute to medication errors Assessment Idea Quiz: Likert scale questions for self-assessment that will assess degree to which the learner fails to comply with safe practices for medication assistance rated from Never to Always. Absorb Activity Presentation: Short slide show to discuss influence of human factors on medication errors. Reading: Provide a resource page to include links to Intranet and other websites on safe medication practice and the health care aide role in medication assistance. Do Activity Connect Activity Reflect on current practices and attitudes about medication assistance and answer 5 questions using a likert scale to assess the degree to which complying with safe practice is challenged by personal beliefs. SEE ASSESSMENT SAMPLE Considering all content from this instruction, use the organizations personal development plan template to write down 3 to 5 ways to prevent medication errors in the coming year. The learner will write down 3 to 5 ways he/she will prevent medication errors in the coming year. Discussion with manager to review compliance will occur during annual performance evaluation.
