Pre-Employment Medical

Dear Flight Personnel, Should you successfully pass day 1 of your assessments with Jet Airways, you will be asked to stay an additional 2 days to complete the pre-employment medical procedures. Per Jet Airways instructions, please print this document and bring it with you to assessments as it contains important information about the ensuing medical procedures. Pre-Employment Medical Procedures Please note that you will need to write your full details (name, date, medical history, etc) on the four (4) pre-employment medical forms which Jet Airways will give to you AFTER your simulator check in BOM. You will need to write the dates as per the below order: Day 1 Screening Day 2 Jaslok Hospital (all the routine tests i. e. CBC etc ) please write the appropriate date on this letter based on Day 2 of your assessments Day 3 Breach Candy Hospital ( TMT test stress test ) please write the appropriate date on this letter based on Day 3 of your assessments All the captains need to undergo medical test at Jaslok, the next of clearing the screening and medical TMT test at Breach Candy the following day of Jaslok, the order of premedicals is given above. Kindly note, all the candidates need to carry 2 passport size photographs (one is required for the premedical form, let them have one extra photograph in case required) and we need to have a copy of their last medical done sent to me which I need to forward to the medical department Jet Airways ( latest medical copy will normally be attached with security clearance docs anyway so no need to send it separately ). For blood test, overnight fasting is required. Candidates cannot have anything except water after 2100hrs previous night. (Blood test will be at Jaslok hospital). 1. Photo - identification paper - first half of the form has to be filled by the candidate (candidates need to stick one photograph).

2. Both Jaslok & Breach Candy letters have to be signed by candidate and write appropriate date on letter 3. Pre employment form first page has to be filled. Jaslok hospital has agreed for cash free transaction hence all bills to be directed to company. When at Jaslok, the captains only need to submit a Jet Airways letter and premedical forms which will be handed over to them by our driver the earlier day. Breach Candy will require cash for TMT test to be conducted. Advance money for TMT test ( I e Rupees 3000 ) will be sent with our driver for the candidates appearing for screening along with respective forms and Jet Airways letter for Breach Candy. The forms will be handed over to captains (who have cleared screening only) post screening the same day in the evening or they will be sent to their hotel and they need to collect the same from our driver who brings it. Premedical test payment receipt at Breach Candy hospital has to be handed over to the driver to be submitted to Ms. Swapna Awasthee at Siroya Centre (i.e. Jet Airwaysoffice) since we need the same for the captains advance payment clearance. Please note, it is very important to retain the receipt as the same is required by Jet Airways.
