4. STEP-BY-STEP DETAILS OF THE LIFTING OPERATION Description 1. Trainer to explain, demonstrate delegates to practice the task as per describe below. Refer practical exercise diagram layout 2 for references: 2. Prepare a lifting plan to include all the key element and approved by the supervisor. 3. Prepare a Job Hazards and Risk assessment for the task 4. Conduct the ToolBox Talk (TBT for all the crew involved). Crane Operator must attend the toolbox meeting 5. Pre use check for the Crane assembly, parts by the crane operator 6. Perform operational check for the Crane by the Crane Operator (boom angle, length, radius, load chart capacity, limit switch etc,) 7. Conduct pre use check for 2 pieces of 16 tons wire rope slings 8. Examine and determine the weight of the load (13000 kgs) 9. Attached the wire rope sling to the load (Gas Metering) by using double choker hitch at lifting point at approximately 25% of total length from both end 10. The chocker hitch shall be secured to avoid damage to the wire rope by using bulldog grips 11. The load shall be attached with 2 taglines placing at the chocker hitch point by using bowline knot 12. Banksman to communicate to Crane Operator informing that the load is ready to be lifted 13. The Crane Operator shall swing to the direction of the load following the direction of the banksman 14. Banksman to communicate by using hand signal to indicate which type of block to be used (Used main hoist)

15. Banksman to communicate with the Riggers that they are ready for the lift and ensure everyone is at the correct standing position. 16. Banksman to direct the crane operator to lower down the block by using hand signal (used hoist/block down hand signal) 17. Once the block is down at approximately chest level, the banksman shall instruct the Rigger to attach the wire rope slings to the crane hook 18. The banksman then will direct the crane to hoist up slowly until the sling is tension. He then must conrm that the sling angle and centre of gravity (CoG) 19. Once the CoG is conrmed, banksman to instruct the Crane Operator to hoist up the load at the same time 1 Riggers to control the load using the taglines 20. Banksman to direct the crane by hoist up signal until the load is 5 metres high. He then use boom down and hoist up signal until the load is above point B 21. Hoist down the load through the cargo hole at point B. Rigger must control the load using taglines only until it safely landed at ground lay down area. 22. Rigger to unhook and de rigging the lifting accessories 23. Perform post use inspection for all lifting equipment to ensure all are free from defects. Any faulty equipment found must be reported and recorded 24. Perform good housekeeping at the work area and send all the equipment back to store. Defect equipment shall be kept at a quarantine area 25. Any waste during the rigging activity must be disposed and segregated as per company procedures
