This letter is prompted by the Commission's preliminary review oI the report reIerenced
above. This notice requests inIormation essential to Iull public disclosure oI your
Iederal election campaign Iinances. Failure to adequately respond by the response
date noted above could result in an audit or enforcement action. Additional
inIormation is needed Ior the Iollowing 2 item(s):
1. Schedule A oI your report discloses one or more contributions that appear to
be Irom a corporation(s) and/or labor organization(s) (see attached). 2 U.S.C.
441b(a) and 11 CFR 103.3(b) prohibit the receipt oI contributions Irom
corporations and labor organizations unless made Irom separate segregated
Iunds established by the corporations and labor organizations.

II any apparently prohibited contribution in question was incompletely or
incorrectly disclosed, you must amend your original report with clariIying
inIormation.

II you have received prohibited contributions, you must make a reIund within
30 days oI the treasurer becoming aware oI the illegality oI the contribution.
(11 CFR 103.3(b)(1) and (2))

II you have not already done so, please inIorm the Commission oI your
corrective action immediately in writing and provide a photocopy oI any reIund
checks. ReIunds must be reported on a Schedule B supporting Line 20(a) oI the
report covering the period in which the reIund is made. (11 CFR 104.8(d)(4))

Although the Commission may take Iurther legal action concerning the
acceptance oI prohibited contributions, your prompt action to reIund the
prohibited amount will be taken into consideration.

2. Commission Regulations require that a committee discloses the
identiIication oI all individuals who contribute in excess oI $200 in an election
cycle. (11 CFR 104.3(a)(4)(i)) IdentiIication Ior an individual is deIined as
the Iull name (initials Ior Iirst or last name are not acceptable), complete
mailing address, occupation, and name oI employer. (11 CFR 100.12) Your
report discloses contributions Irom individuals Ior which the identiIication is
not complete.

The Iollowing employer and occupation entries appear on your report and are
not considered acceptable: "InIo Requested/InIo Requested."

You must provide the missing inIormation, or iI you are unable to do so, you
must demonstrate that "best eIIorts" have been used to obtain the inIormation.
To establish "best eIIorts," you must provide the Commission with a detailed
description oI your procedures Ior requesting the inIormation. Establishing
"best eIIorts" is a three-Iold process.

First, your original solicitation must include a clear and conspicuous request Ior
the contributor inIormation and must inIorm the contributor oI the requirements
oI Iederal law Ior the reporting oI such inIormation. (11 CFR 104.7(b)(1))
See 11 CFR 104.7(b)(1)(B) Ior examples oI acceptable statements regarding
the requirements oI Iederal law.

Second, iI the inIormation is not provided, you must make one Iollow-up, stand
alone eIIort to obtain this inIormation, regardless oI whether the contribution(s)
was solicited or not. This eIIort must occur no later than 30 days aIter receipt oI
the contribution and may be in the Iorm oI a written request or an oral request
documented in writing. (11 CFR 104.7(b)(2)) The requests must:

clearly ask Ior the missing inIormation, without soliciting a contribution;
inIorm the contributor oI the requirements oI Iederal law Ior the reporting oI
such inIormation, and
iI the request is written, include a pre-addressed post card or return envelope.

Third, iI you receive contributor inIormation aIter the contribution(s) has been
reported, you should either a) Iile with your next regularly scheduled report, an
amended memo Schedule A listing all the contributions Ior which additional
inIormation was received; or b) Iile on or beIore your next regularly scheduled
reporting date, amendments to the report(s) originally disclosing the
contribution(s). (11 CFR 104.7(b)(4))

Please amend your report to provide the missing inIormation or a detailed
description oI your procedures Ior requesting the inIormation. For more
inIormation on demonstrating "best eIIorts," please reIer to the Campaign
Guide.
Please note, you will not receive an additional notice from the Commission on this
matter. Adequate responses must be received by the Commission on or beIore the due
date noted above to be taken into consideration in determining whether audit action will
be initiated. Failure to comply with the provisions oI the Act may also result in an
enIorcement action against the committee. Any response submitted by your committee
will be placed on the public record and will be considered by the Commission prior to
taking enIorcement action. Requests for extensions of time in which to respond will
not be considered.

Electronic Iilers must Iile amendments (to include statements, designations and reports)
in an electronic Iormat and must submit an amended report in its entirety, rather than
just those portions oI the report that are being amended.
