1. Confirm that the questionnaire identifier on the questionnaire booklet cover page matches the questionnaire identifier on the answer sheet. If they do not match, contact your questionnaire administrator. 2. At the top of the answer sheet, print the required information in the spaces provided. 3. Begin the questionnaire. Read each question carefully and then select the response(s) that comes closest to describing you. Mark your response(s) by writing it in the area designated on the answer sheet. 4. There is no time limit; work at your own pace. 5. Do not skip any questions. 6. If you want to change a response, erase or cross out your original response and then record the correct response. 7. Please do not make any marks in the questionnaire booklet; it will be used again.

A financial services sales career can be challenging but also very rewarding. The Career Choice questionnaire is designed to help you understand your success potential in this career. Career Choice consists of 49 items and should take you about 15 minutes to complete. Please read each question carefully before responding. Your honest responses will provide you with a more accurate idea of your success potential in this career.
