DISABILITY SERVICE EXAM ACCOMMODATION PROCEDURE Disability Service coordinates exams for students with disabilities who are

not able to write with their class. This may include students who are entitled to accommodations such as use of adaptive equipment, a scribe, a distraction free environment or extended time to complete the examination. While these arrangements are designed to ensure that appropriate disability related accommodations are in place, all of the Universitys academic and examination regulations apply. ARRANGING EXAM ACCOMMODATIONS The Disability Service Coordinator and the student determine the type and extent of accommodations required based on the students disability documentation. After the student makes arrangements for exam accommodations with the Disability Service Coordinator, s/he is asked to promptly discuss these arrangements with their instructor(s). The following steps are to be followed by students writing their exams with Disability Service. 1. As soon as the student has been notified of the exam, s/he is to pick up a Request for Exam Accommodation form. If an instructor provides the dates of all in-class, mid-term and final exams at the beginning of the term, the student can complete all the forms for each of these at the same time. 2. The student will contact the instructor to discuss the exam accommodation. The student will ask their instructor to check and sign the form. 3. 4. Forms are returned to Disability Service at least one week prior to an exam. A copy of the form will be provided to the instructor. The instructor will deliver the exam to the Disability Service Coordinator prior to the start time of the exam. ON THE DAY OF THE EXAM 1. Students should try to arrive 10 minutes early and be ready to begin the exam at the designated time. Start and stop times for exams will be strictly followed. 2. The only material a student is allowed to take into the exam room will be the material that has been authorized in writing by your instructor. 3. Coats, jackets, and bags are not permitted in the exam room. It is preferable that these items are left in a locker. A location will be provided for any items that are not permitted. 4. The Disability Service Coordinator will assign the student a room (alone or with other students) with an invigilator present or will assign the student a private space with periodic checks by an invigilator. AFTER THE EXAM All examination materials will be collected and passed on to your instructor by the exam invigilator.

DISABILITY SERVICE REQUEST FOR EXAM ACCOMMODATION This request will not be supported unless you have prior approval on file with the Disability Service Coordinator for exam accommodation. It is the students responsibility to give Disability Service a minimum of two weeks notice to book an exam.
