Attention 1: Dr Ezim Obineche Koon Poh Teoh 19 Frobisher Road London N8 0QT 20th January 2014 Re: Repeat prescription ; I object to Free NHS Health Check / Review

Note: Could you please answer the following as numbered. If you disagree to any therein please give a full explanation with supporting evidence and/or specific law applied. Omissions or refusal to answer, would be inferred (implied or expressed) as acquiescence. 1. I refer to the matter above in respect to Mrs Velans undated letter and your subsequent message via a note attached to the prescription to me which I received on 08/10/13 and 11/10/13 respectively. I noted they are unlawful as it is part of an arranged scheme against me. I can see that you aid, abet and assist the principal perpetrators. Please see Note above. 2. In accordance with the Surgery print-out for repeat prescription I need only to give you 48 hours each time I make a request. I did tick FOUR ITEMS. The list was sent to you first class on 18/11/13. However, when I turn up for collection on 22/11/13 I encountered an unfriendly resistance to my request. I was appalled by the resistance, tricks, schemes applied despite my friendly assurances. It appeared someone is behind this premeditated plan. Please see Note above. 3. From what transpired, the acts and omissions indicated plainly and clearly there is intention not only to deprive but also to change / alter my normal prescription without my consent, agreement or my knowledge. I was given only two out of four items I requested: Adcal-D3 750mg / 200unit caplets ( ProStrakan Ltd) and Fluticasone 125 micrograms /dose Salmeterol 25 microgram/dose inhaler CFC free. Please see Note above. 4. I was deprived of two items. (i) Cetirizine 10mg tablets. OD, 2X30 tablet(s) & (ii) Vitamins capsules Take one daily, 100 capsule(s). As a result I have fallen ill with infections that affected my asthma and thereafter chest pain. This is an injury in fact. This is exactly what you; the said healthcare providers did to me. You did not provide what I need. Is this injury for my best interest? Please see Note above & Para.2 & 3of your letter. 5. I also refer to your letter dated 27/11/2013 which is clearly out of time, out of touch and wrong despite my explicit letter dated 18/11/13 and further explanation on 22/11/2013. As a competent adult and in my capacity as a qualified professional I do know what is right, wrong and unethical. The 4 items I requested on 18/11/13 per 3. & 4. above is common place and had been agreed and prescribed due to my condition and circumstances. Please see Note above. 6. From my observations I can inferred that the Free NHS Health Check by a nurse is a pretext for an intended Review as confirmed via your message and letter. Please see Note above. 7. Please find enclosed my order for a repeat prescription. I will keep to the 48 hours as set out and will collect as soon as possible. Note: I object to any change / alteration whatsoever to the agreed prescription (e.g. seritide inhaler) without my prior consent or agreement. I therefore urge you to dutifully blow the Whistle 2 loud for yours and the Public interest. 8. Please examine carefully the contents of this letter and inform me of any discrepancies or disagreement whatsoever giving full explanation with specific law applied within SEVEN DAYS from the date of this Notification, failing which after a further 7 days had expired, this Notification and its full contents would be deemed accurate, correct and legally binding. Whistle blower - A worker must not be subjected to any detriment because he has made a protected disclosure [see S. 47(B) AND S 48 (IA) Employment Rights Act 1996 (inserted by the Public Information Disclosure Act 1998).

All acts/omissions and correspondence addressed to me must be legal otherwise it is unacceptable and I object to it.
