Change the name on your driving licence from GOV.UK
Com plete the right form and send it to DVLA with your old driving licence and any supporting docum ents.
Photocard driving licence

include your photocard driving licence and paper counterpart
You dont need to send a new photo.

include your driving licence and a passport-type photograph
If you cant send in your old driving licence
You m ust declare on the form if your old licence has been lost, stolen or destroyed.
Use the processes above to change your title (eg Ms to Dr). You dont need to send evidence for the change unless its for an hereditary title.
When to expect your licence
DVLA aim s to deliver your new driving licence to you within 3 weeks of getting your application. Itll take longer if they have to check your health or personal details. Allow at least 3 weeks for your licence to arrive before contacting DVLA.
If youre disqualified from driving
You can still tell DVLA if you change your nam e while disqualified. Write to DVLA, Swansea, SA99 1AB giving details of your old and new nam e, your driving licence num ber (if known) and date of birth. Your driver record will be updated and youll receive a D27 driving licence renewal application 56 days before your disqualification ends.
