Classification and Mechanismm of Action

Indications -Edema associated with heart failure, hepatic and renal disease

Side Effects -CNS: asterixis, dizziness, weakness, headache, drowsiness -CV: osthostatic hypotension -GI: nausea, anorexia, vomiting, diarrhea -GU: polyuria, nocturia -Hematologic: hypokalemia, leukopenia, anemia

Contraindications -Contraindicated with allergy to bumetanide or sulphonamides; electrolyte depletion; anuria; severe renal failure and hepatic coma -Use cautiously with gout and diabetes mellitus

Nursing Responsibilities -Assess vital signs before giving medication -Take the meds with food or meals to prevent GI upset -Give the drug early in day so increase urination will not disturb sleep -Provide diet rich in potassium or supplemental potassium -Avoid use if oral use is possible -Instruct patient they may experience increase volume and frequency of urination

Monitoring Parameters -Monitor serum electrolytes, hydration and hepatic function during long term therapy -Monitor weight of the patient

-Loop Diuretic -Inhibits the reabsorption of sodium and chloride from the proximal and distal renal tubules and the loop of Henle, leading to a natriuretic diuresis.

Classification and Mechanismm of Action

Indications -Treatment of adults patient with type 2 diabetes mellitus who require basal insulin control of hyperglycemia

Side Effects -allergy local reactions at injection site -Metabolic: hypoglycaemia, ketoacidosis

Contraindications -Contraindicated with allergy to pork products, history of smoking or lung disease -Use cautiously with gout and diabetes mellitus

Nursing Responsibilities -Assess vital signs before giving medication -use same type of syringe; same type and brand of insulin to avoid dosage error -Rotate injection site regularly to prevent breakdown at injection sites -Store drug in the refrigerator or in a cool place out of direct sunlight -Avoid alcohol; serious reaction can occur -Report fever, vomiting, hypoglycemic or hyperglycemic reactions, rash

Monitoring Parameters -Monitor urine or blood levels for glucose and ketones as prescribed -Monitor injection site regularly
