2 CLINICAL OVERVIEW

The purpose of the clinical sessions in this course is to provide you with an opportunity to work with older adults who are not acutely ill. You will conduct a thorough interview with an adult 70 years of age or older to assess for needs that may impact the persons health, provide interventions that would promote keeping him/her at home and healthy, implement the interventions, then evaluate for effectiveness. You will observe how people cope with their physical problems of aging, how they modify their environments, and how families and older adults deal with psychological and cognition difficulties. During your interview they may describe their life and how they deal with the aging process, steering you into wonderful laughter and joy as well as tearful moments as you feel their sorrows. After interviewing older adults, you will appreciate their uniqueness and contributions, and how each is unique in their ability to adapt to the changes that accompany aging. All of your experiences, interview summary, and your interventions will be shared with your client. Clinical policies: Attendance at clinical lab meeting is required to pass N410: You must get a satisfactory grade and fulfill all outcomes in clinical to pass the course. Absence of clinical meeting should be informed to the clinical instructor in advance and should be covered by the guidance of the instructor. If you must miss a clinical session, you will be assigned a topic for a five-page minimum professional paper that will be due two weeks from the missed session. Unsatisfactory clinical performance or absence of clinical conference more than once will result in a failing grade for the entire N410 course. Your clinical faculty member has total authority and responsibility to remove you from clinical practice if your behavior is deemed unsafe or unprofessional. All Clinical Outcomes must be COMPLETED to PASS the Course. Clinical Meeting Times and Places: Follow the schedule of each clinical session. Liability, Vaccinations, CPR Certification, Course Field Trip Students must submit a copy of proof of compliance to their clinical instructor and submit the Course Field Trip form on the first clinical meeting. Dress Code in Clinical 1. Picture ID badge must be visible 2. Professional dress for clinical requires street clothes unless the clinical site specifies. 3. Follow Dress Code, Clinical Attire and Hygiene Guidelines on the Undergraduate Student Handbook at http://nursing.sdsu.edu/miscfiles/undergradhandbook.pdf. Under no circumstances are jeans, t-shirts, visible midriffs, flip-flops, and high heels acceptable. Any student considered unprofessional in appearance will be asked to leave the clinical setting. This will result in an unexcused absence and will affect the students clinical evaluation.

Each student has a responsibility to manage the clinical work on his/her own time schedule. Some clinical experiences (i.e. hospice, dementia care facility, and community service) and all individual activities must be accounted for in your journals. See the clinical journal guidelines. You may not miss classes or clinical in other courses to fulfill N410 course requirements. Clinical instructors have authority to determine what activities count as clinical hours (see below for alternative clinical sites). All Clinical Outcomes must be completed by the clinical due date.

Interview, Gerontological Assessment, and Nursing Interventions 1. Each student will interview one older adult over the semester and assess the health conditions of the person. In each interview, students apply different functional assessments to the person and provide feedback and nursing interventions. 2. The purposes of this assignment are for students to 1) improve their communication skills with older adults, 2) gain empirical experience to assess normal age-related changes and their effects on the functional ability of older adults residing in the community, and 3) develop interventions/teaching plan based on the empirical findings and theoretical foundations. 3. Selection Criteria: Adults who (1) are age of 70 years or over, (2) have at least two medication prescriptions (excluding PRN medications), (3) are able to communicate clearly, (4) are available to meet the student in person at least four times by appointment, and (5) agree to answer and ask questions regarding his or her health and life for the benefit of the students learning. 4. Exclusion Criteria: You will exclude older adults who (1) suffer moderate/severe dementia, (2) are undergoing acute care, (3) are admitted to acute/skilled nursing care facilities, and (4) live in outside San Diego County. 5. Guideline of Interview Interviews are allowed only in person and not by phone, video recording, or any other on-line device. Please do KEEP the participant while the clinical course is underway. If you must change the participant, you should repeat the interview process with the new participant up to the time of the replacement. Before using the assessment questions or tools, please READ carefully and understand the meaning of the questions, tools and the related passages in the textbook. If you cannot understand the content, please feel free to ask your clinical instructor. You will assess the older adults health based on the participants subjective reports and your objective observations including nursing knowledge. While you are assessing your participants health, you need to consider the participants tiredness. You may postpone completion of the questionnaire until a later time. You may participate in your client's usual activities during the interview hours. All the information obtained from your interviewee will be kept CONFIDENTIAL. Instruction for Interview For the first meeting Find one older person that meets the inclusion criteria from your family, neighbors, senior center, senior apartment, etc. Older adults at an assisted living facility may not be qualified for this course work. Introduce yourself and invite the older person to participate in the interview. Fill out the Interview Agreement including both parties phone numbers: Leave one copy for your client and submit the other copy to your instructor. Set a meeting place, date and time for the next meeting (Interview part I).

Request that your client bring the medication information (name, dosage, frequency, etc.) to the next meeting for Interview part I

Client Interview Part I Interview your client using the form, Interview Part I, within the allowed time (2 hours). It is very important to obtain an accurate medication history and 24 hour diet recall found in the Part I Interview. After your client interview, analyze the data collected, identify all nursing diagnoses, and develop your teaching materials. Submit this information at the second clinical conference meeting with your instructor. Client Interview Part II Provide the results of 1) the analysis of medications, 2) 24 hour diet analysis, and 3) 10 year fracture risk calculation to your client and explain the result and interventions. Interview your client using the form, Interview Part II. Discuss the possible nursing diagnoses with your client and select the best available one upon the clients agreement/willingness. Client Interview Part III Interview your client using Interview Part III Implement your planned nursing interventions and observe your clients response After interview III, contact your client and ask about your clients response to the interventions. At the last meeting Provide your client with the summary report and explain the content. Conclude your meeting with your client. Interview Topics:

Grading Criteria Interview and Intervention Credit: 60 points The quality of the functional/health assessment obtained by interview will be evaluated by the instructor based on the accuracy and completeness of the data, the feedback to your client, and intervention related to the nursing diagnosis. Accuracy: When you interview older adults, several points are helpful for getting valuable and objective information: (1) obtain direct information as expressed by the older adult, (2) do NOT force your opinion on the participants answers, and (3) add your objective information in a comment section. Completeness: Answer all questions including comments and feedback of each assessment. Feedback to your client: When you get information from your client, give feedback to her/him about the normal range of the assessment, what it means regarding the clients health status, and how the situation can be improved. Write what you provided to your client in the comment section at the end of each instrument. Teaching and interventions: From the findings you obtained from the interview tools, you may need more detailed information than that obtained from the tools. For example, if a person has a chronic pain, ask about the condition in detail such as severity, frequency, duration of pain, influencing life style (activity, sleep, etc.), how the participant manages it, medications to relieve the symptoms, and whether the management was effective. With this information, you can set a few realistic interventions to improve the condition. Provide references regarding your information (nursing diagnosis, intervention, and educational materials). Credit: Refer to Grading Criteria for Clinical Activity on p. 14 Medication and Nutritional Analysis: 1. Submit his/her BMI, two types of nutritional analysis. 2. Medication analysis: Make a table with at least 14 fonts or higher. This table is for your client, so explain in laymans terms. Make it simple and efficient in space using. A Summary of Interview and Nursing Care Credit: 30 points Purpose: The results of your interview will be summarized in a final report to give your client. The assessment, findings, nursing diagnosis, interventions and teaching should be reported. The report should be written in lay terms for your client to understand. Introduction (8 pts): Start with narrative description directed to your client that includes the purpose of this paper, place and dates of interview, demographic information (age, gender, race, living status, etc.), health/medical conditions, and number of prescriptions. Briefly describe his/her findings in each instrument (client scores and normal ranges). 1 ~ 2 pages of length Synthesize data: (8 pts): Determine the one or two highest priority nursing diagnoses with rationale. Describe your concerns about your clients health and provide rationale (This is a foundation of nursing diagnosis). Explain why you concern the health issue with all objective and subjective information. o This information can include but is not limited to the clients expression (subjective) and your observation (objective) and nursing/medical knowledge (i.e., medical diagnosis, nutritional or behavioral habits, assessment scores by the instrument, etc.). o Consider that your client is a whole person. You should include all, physical, social, or emotional issues (e.g. habitual behavior, economic status, living condition, etc.) and not just focus on medical problem(s). o You should take the facts from the assessment section and synthesize them into a pattern of what may be causing difficulty for the client, or where his/her knowledge could be improved. You can repeat the information described in the introduction section. Provide what your expected health outcome from your intervention is. You can reflect on what your client wants to improve/learn. 1 ~ 1.5 pages of length Interventions and results (8 pts): Summarize what you provided to your client to improve his/her health status and the result of your intervention. Describe interventions about what you provided or practiced. Be specific about exactly what you provided for your client. Example: Taught to practice strength/chair exercise for 10 minutes daily. Reduce the amount of carbohydrate servings to six per day. o Do not need to provide general/overall information, which may be needed for you but not for your client in this summary paper. Describe what kind of method was used for delivery of your interventions. Describe how much your goal reached. Specify the result of your intervention. o For example, 1) if you provided some intervention to reduce her/his blood pressure (BP), pain, body weight, etc., show the result of BP, pain level, body weight at the beginning and end of the semester. Bring the Clinical Hours Log to get the staff information whenever students visit any facilities. Submit a clinical journal entry within one week after each activity. The following places are recommended to visit and serve. If any students want to visit other than the listed places, approval from the clinical instructor is needed in advance. Provide the characteristics of the institution. Write a Thank-you Note to the staff who was an excellent mentor during your clinical visit.

Palliative and Hospice Care Facilities Each student is required to visit one day (8 hours) at one location. The schedule for hospice (site and day) is posted on Blackboard, Combined CX-BB, at the first week of the semester. Each student has a responsibility to manage the hospice schedule with other course work (i.e., clinical meeting days, individual activities, classes, etc.).
