I. INTRODUCTION Each one should use whatever gift he has received to serve others, faithfully administering Gods grace in its various form 1 Peter 4:10 Throughout the course of history, service has been observed and predominant in many places and many times. It has been a manifestation of mans innate character to serve others. Service has been shifting in various forms and wearing different countenances a man lending his helping hand to his neighbor, family members loving each other, and Jesus giving His life to His people, the ultimate sacrifice and service a man could ever do to humanity. The Philippines is fortunate to be one of the worlds birthplaces of the concept of community service. Early pilgrims have to serve each other to keep from dying in early times. When the early colonists realized they had a choice between tyranny and freedom, they banded together in every community and form minute men or militia to keep their communities safe from the tyranny of Spanish military oppression. Men, women, and children with deep conviction and high integrity rose to face the difficulty of the times with heroic character. Lyceum of the Philippines University Batangas had thought of ways to serve and contribute to the alleviation of humanitys predicament. It exists not only to provide quality education but it is also committed to community extension programs and human welfare developments. These are beyond the institutions obligations but LPU-B makes it possible for students to lend a helping hand to others. One of the colleges of LPU-B is the College of Nursing. One of its goals is to promote initiativeness and generosity among its studentry. The formulation of community duties provides a room of fulfilment for this goal. This community service lets a student perceives realities of life: him being a part of the community, existence of problems in the community and involvement of himself in the solution of those problems. As a component of the nursing curriculum, the Bachelor of Science in Nursing IV-2 GROUP D was assigned to the community of Barangay Sto. Nino, Batangas City. With the help of certain officials, ocular survey, community interview and over-all community assessment were done in order to know the status of the area. Through the use of the family health care process, the students adopted a family and evaluated the health status of the family members.

After assessment, the student prepared pamphlets and other instructional devices aimed towards information dissemination about current health lifestyle habits in order to develop the health status and improve the quality of life in general. The health teaching implemented by the student is geared towards improvement of current lifestyle habits in order to prevent the acquisition of diseases in the future. Through gradual learning of the community nursing students can be able to define each vital components of the community and the role of each in community building. Through Related Learning Experiences embarked in the nursing curriculum, the student is exposed to a real community in which he can be able to apply his knowledge and learning to this setting. Moreover, a nursing student is encouraged to step out of his comfort zone to see and to do things beyond his capabilities. With every community service activity or project comes the explicit or implicit goal of improving the lives of individuals in the community. Community service efforts are designed to help individuals and /or groups deal with various issues confronting our society. Also for every act of service there is a ripple effect that ultimately results in some degree of benefit to the entire community. Lifes richest moments are those we share with others. Family and community are central to a meaningful life, therefore, the values of diversity, mutual respect, and social responsibility should apply not only with our own walls but also within a broader social context. We, students, must have been taught that the gifts of learning, leadership and preparation are given so that they may be shared with others. Because the greatest gift of all is to know that you made a difference in the lives of those who need it most. II. Photo Documentation

III. Brief History of Barangay Sto. Nino, Batangas City

According to the elderly, a very long time ago Barangay Sto. Nino was formerly called Buayaha because their river is being inhabited by a lot of crocodiles. And this has brought fear to the people of the barangay, because of that they sought help on how to get rid the crocodiles off their river. Because of their belief that they can be Patron Sto. Nino, the people of the barangay annually held a ritual at the deepest part of the river called Batalan. In this certain part of the river they were accompanied by their Queen of the fiesta with the members of the event together with the people of the barangay. They let their Patron Sto. Nino floats by the river and catch it afterward. By catching their Holy Sto. Nino, they believed that they can get rid the reptiles off their river. A long time has passed by doing the same rituals over the years and the people woke up without the crocodiles on their river. They have no idea on where the reptiles have gone, many have said that they migrated on other place and died. Because of this occurrence, the fear that has been felt by the people of the barangay was gone. And they thank the Almighty God for this and named the place after their Patron Sto. Nino who is now believed to be the reason on the disappearance of the crocodiles living in the river of their barangay.

Barangay Sto. Nino is hilly and mountainous with a total land area of 48, 657.91 square kilometers and located far from the city. The major source of livelihood was farming and livestock raising. Other major sources of livelihood include employment (local and abroad) and retail stores or sari-sari stores. IV. Integration with Community People

Students are given the opportunity to integrate with the Sto Nio community. In this regard, the group were able to gain knowledge as well as experiences while learning to appreciate and acknowledge all things in the everyday life. The group did not just lend a helping hand for the people in the community, but they also transformed themselves in all aspects of their being, may it be physical, intellectual, emotional, social, or spiritual.

As part of data gathering, the group interviewed some families using the prescribed tool.They targeted the depressed families in order to determine the problems present in the community.The group had interviewed 35 families. All of which are living in Brgy. Sto Nio,Sito CentralBatangas City. Family Structure, Characteristics, Dynamics and Socio-Economic

The graph shows that 29 percent of the occupations of the head of the family are OFW, 28percent are driver, 25 percent are laborer and 18 percent are farmer.

The distribution of community people in Sto. Nino, Batangas represents that 47 percent of the population are female, which is the largest percentage of the total number of population live in the community. Second is the male person, you can see in the table that 40 percent of the total population is male, while 7 percent of the population are range in 0-7 year old are female and 6 percent are range in 0-7 year old are male.

This pattern of the population distribution in the Sto. Nino, Batangas has major implications for the community development. These sparsely populated areas are due to the fact that most of the community people are female rather than male. Home and Environment

On the bar graph being presented it was seen that among all of the community people that was interviewed at SitioCentral , most of them owned their houses which has a total of 31 household and 3 of them were renting their houses. Also there is a total of 28 households who owned their lot and 6 of them were renting their lot. And during the interview it was observed that that 13 household were made of both wood and concrete, 7 of them were made of wood only 3 houses are made of pure concrete. The source of water at Barangay Sto.Nino, Sitio Centro was mostly through the use of waterworks which has total of 26 household using the water system, 7 of them were using spring as a source of water and only one household were using deep well.

For the cooking facility, 51 percent of the households of the Sitio Central are using LPG as their instrument for cooking. And about 31 percent of them use woods as a tool for cooking, 16 percent were through the use of charcoal and only 2 percent household use electric stove for cooking. And for the electricity, the whole community utilized electricity was the primary source of their power supply.

Base on this graph being presented it can be seen what appliances are being utilized most on the 35 households that was interviewed. From this data it can be seen that 29 of all the households are using television, 25 radio, 27 electric fan, 16 refrigerator, 15 washing machine, 7 computers and 1 of them uses iron as an appliances at their home.

Most of the people at the community does not own vehicles but 33 percent of them own a motorcycle, 29 percent own a jeepney, 24 percent of them own a car/van, 9 percent own a tricycle and only 5 percent of them have bicycle.

Every community and households owned animal which are being used for work, source living and even as a pet for entertainment and satisfaction. Base on the Pie Chart being presented above it can be seen that most of the households own dogs which is composed of 42 percent , followed by cats as pet which has 20 percent, 20 percent of rooster also used for cockfighting, 8 percent were goats, 5 percent were pigs as a source of living and only 2 percent for rabbit and cow.

It can be seen on the Pie chart above that Sto. Nino, Sitio Central utilizes four ways of disposing their wastes. Most of them dispose their garbages through burning of wastes which has a total of 61 percent, 30 percent of them of them dispose their wastes through compost pit, 3percent of them bury their waste, and 6 percent of them segregate their wastes before disposing.

The people at Sto.Nino, Sitio Central have 71 percent of open drainage and only 29 percent of them stated that they have closed drainage. Socio -Cultural The relationship between members of the family was good and sometimes being tested which indicates that they face the normal ups and downs of life. In this dark side of their lives the head of the family and elderly are the one who settles whatever conflict they encounter. Institution Present in the Community There are identified institutions in the community. It includes two (2) elementary schools located in the upper hill of Sitio Centro of the barangay. They have 20-300 teachers working in the school. They also have a high school in the nearby area. Teachers working here are approximately 13-17 in numbers. Most of the students they cater are native of the barangay both elementary and high school. Other students came from neighboring barangay. There is also a health center inside the multi-purpose hall and where nurse, midwife and barangay health workers provide health services. Among the services given are for pregnant women, for well babies/children, for elderly, family planning programs, nutritional and dental program. Most of the community people states that the father of the barangay ( their barangay chairman ) as well as his councilors portrays a good leadership and governance. Some of the programs of the barangay focus on peace and order, health, education, environment culture and sports. A chapel is also present in the community where people gathered and celebrate mass once a month. If theres any important or minor announceme nt

Whenever we are seek we all have our health seeking behavior, in such ways for us to get better from our illness. Base on the result of our interviewed it was discovered that the people of the community only 18 of them stated that they go directly at government hospitals whenever sick, while 16 of them stated that they consult and utilize the health center. There are also those who go to the private hospitals and there are also people there who still believe in the power of faith healing or the ability of the quack doctors.

On the data presented above it can be seen that most of the community people seek medication at their health center. It was followed by taking medications through physicians prescription and by the use of herbal medicines. There are also those who claimed that they prescribed medication to themselves when sick.

Every community are never immune to acquiring different types of illness or diseases such as diarrhea, cough and cold, fever, malnutrition and even hypertension. And there are different ways in acquiring this certain diseases, on the data presented above it can be seen that 75 percent of the community stated that they acquire such illnesses from the pollutions in their community, 15 percent of them stated that it is caused by ingestion of contaminated food, 6 percent due to have inadequate knowledge on proper preparation of foods, and only 4 percent stated that this is due to the change of weather

When asked about the health promoting activities that they practice, most of them perform such health measures. These activities are by doing household chores and exercising every day, some of them also stated that they provide enough rest and sleep for themselves, and enough leisure time and relaxation activities. They also claimed that they eat nutritious foods every day to be protected from diseases.

It was seen from the data collected that the people of the community practice such health measures to prevent illnesses and diseases. This are through the use of proper hand washing, but the use of mosquito net, rainbow and umbrella, by proper disposal of wastes and through provision of well sanitated toilet facilities.

VI. Participation in Health Unit and Other Community Activities

With the Brgy. Health Center midwife together with the BHWs The group was able to participate in other community health activities like immunization of child group and prenatal checkup for pregnant woman.The group was allowed to administer vaccines like OPV, Hepa, DPT and measles .The barangay health workers allowed us to do some of vital signs taking as well as the height and weight measuring of the children and pregnant woman.

VII. Looking for a Potential Leaders and Volunteer Community Health Workers

Asking for Potential Leaders that are willing to Participate The group went to different houses in the Sitio Central of the barangay to ask and invite for the community assembly which will be held in the court of the barangay. With that assembly, an organization and potential leaders will be taking place. This leaders will take the biggest part in the community activities which will enhance their community leading to continuous progress. The criteria of a potential leader are willing to work and participate, can share ideas for improvement of the organization, willing to sacrifice time to spend in Community Assembly. We also humbly ask the assistance of some barangay official to invite people to participate in the said assembly and the aim and purpose of the assembly is well explained to the community people.

VIII. Community Assembly

Ms. Magpile explaining the objective of the assembly The group facilitated a meeting regarding Community Organization at Sitio Central, Brgy. Sto Nio, Batangas City last November 09, 2014. The participants were mostly attended by the mothers in the community. The meeting started at around 9:30 am with a prayer led by Mr. Kenneth Mayuga. Here, the community people were asked on the possible and existing problem of their community. With regards to this, different thoughts and views were heard from them.

Ms. Guieb explaining the identified problems in the community. In the middle of the meeting, unfortunately, the people refused to undergo an election of officers who will lead and initiate the possible actions to their identified problems. According to them, discipline is the key to solve the problem. In the end, they decided that each one of them will be the leaders of their own home to achieve their goal with regards to the problem identified. The students respect their decision and resume the next activity of the assembly.

However, the student also did a health teaching with regards to their identified problem. They had a brief discussion on the benefits of the interventions that can be made to solve the problem as well as its disadvantages in relation to not only health aspect but also in general.

Team Building Team building is done to improve collaboration, positive

communication, leadership skills and the ability to work together as a group to solve problems. The team building activity which was done is named Calamansi Relay . The physical characteristics of each one of the individual joined in the game can also be observed as they show their speed, alertness and mobility. They were given instructions and mechanics of the games as well as the violations that should be prevented while playing. This is to promote fairness and honesty in the game.

After the very exciting game of calamansi relay, the students continue to give more surprises to the community people the raffle draw goes next. Before the program starts, the participants were asked to sign in an attendance sheet and it was numbered 1 onwards. The raffle was facilitated by writing the numbers that corresponds to the participants names and put in a plastic container where in the councilor randomly picked.
