Upon approval of applicant's General Facility Use Request or Season Facility Reservation Request by an authorized representative of Denton County Fresh Water Supply District 1-A, a copy of this signed Agreement will be returned to the applicant and must be brought to the park site on the date(s)/time(s) of authorized use to verify the reservation in case of conflict. Applicant agrees that: 1. Park use is for Castle Hills residents only. 2. A team roster must be provided at the time of application. 3. Applicant must show proof of residency in Castle Hills. 4. Park use is for practice only, games use is not allowed. 5. Applicant will comply with any and all opening, closing, and/or use procedures established for the Castle Hills Sports Field Facility and the Rules and Regulations of District 1-A. a. Applicant will report all unsafe conditions, problems with the facility, and/or reservations immediately to one of the numbers listed below. District 1-A will work with the Castle Hills Master Association to resolve all such condition. Applicant is responsible for leaving the facility in clean and orderly fashion, and responsible for any damage to the facilities and or equipment. c. If cleanup is required after use, the applicant will pay a cleanup cost, which will be billed at $40.00 per man hour (this is mandatory and will not be waived under any circumstances). 6. Moving of goals is prohibited and may result in suspension of field privileges. 7. Applicant will use the area in accordance with General and Special Park Rules and Regulations as posted, a copy of which is included herein. 8. Cancellations must be made 72 hours prior to the use date. 9. Glass beverage containers, alcohol, tobacco products, and firearms are prohibited within the Facility. 10. Applicant will inform all team members, parents, or other spectators of all field rules. 11. Applicant will ensure that all vehicles park in the designated parking areas only. 12. Practice will be on the assigned date, time, and field location only. I, the undersigned, have read and thoroughly understand the provisions of this application/agreement and agree to abide by all of the provisions thereof and therein. In addition, I understand that not adhering to the Applicant Agreement Rules and Regulations will forfeit any future use of the Facilities. Applicant Signature: Date:
