Failure of Counterbalance Cylinder Due to Incorrect Installation All Top Drive Drilling Systems

All TDS Counterbalance Systems Advise of Failure and Remind Users of Proper Installation A TDS-3H recently experienced the failure of one of two hydraulic cylinders used in the TDS Counterbalance System. The cylinder tie rods sheared and the cylinder body fell approximately 25 meters to the rig floor. No injuries were reported as a result of this incident. The preliminary report indicated that the top drive had been changed out with another unit and the connection distance between the hydraulic counterbalance cylinders and the becket was not measured and readjusted. As a result the cylinders were fully extended when installed. Note: The weight of the top drive was resting on the guide rail stops and not suspended from the traveling equipment as required prior to assembling connecting hardware attaching shackles, chain etc. When the counterbalance system was pressurized during normal operation the hydraulic cylinders stroked out and were exposed to full drill string weight, resulting in a failure of one of the cylinders.

Solution:

When installed correctly, the counterbalance cylinders will not be at full extension when the top drive picks up the drill string. In order to determine the correct chain length between the traveling equipment and the top drive counterbalance cylinders, refer to the National Oilwell Varco service manual. The procedures in the manual should be followed during installation of the counterbalance system. See Figure 1.
