1. I, the undersigned, hereby authorize the producers to make video recordings, motion
pictures, photographs and/or electronic sound recordings (hereinafter referred to as
photographic or electronic reproductions) of me for the purposes of creating and promoting
the above titled video production.
2. I authorize the use of such photographic or electronic reproductions under terms of the
Creative Commons Attribution-ShareAlike 4.0 International License.

3. I hereby release and hold harmless the Producers and their assigns from claims or
liabilities of any kind arising out of or in connection with the making of or use of such
photographic or electronic reproductions.
4. I understand and agree that I will not be compensated in any way for the use of such
photographic or electronic reproductions or for any other rights granted by my herein, and
that the producers and their assigns may rely on the rights granted herein.
5. I represent that I am at least eighteen years of age on the date of my signature below. If I
am under the age of eighteen years, my legal guardian has signed below, giving their
consent to the foregoing.
