We are planning to operate with this roles: call center operator, street physician and patient. How do you imagine the software within our operational model ? USE CASES RELATED TO PROCESS AND FUNCTIONALITIES a) Patients process 1. New patient creation. Data loaded. Fields creating (eg. "Osdes provider", "relatives", etc.) 2. New patient creation or new diseases added: 1 use case for COPD, 1 use case for diabetes, 1 use case for hypertension, 1 use case for cardiac disease 3. General use of the web portal related to patients, non medical caregivers or other roles: data loading, alerts, social media, networks, message with doctor or call center, information attached, agenda, etc. 4. Example of data loaded through medical devices (spirometer, glucometer) 5. Access to the web portal and functionalities through smartphones and tablets 6. Example of video-consultation 7. Patients quit of the program 8. Examples of SMS functionalities or other telephonic funcionalities (eg. IVR) 9. Examples of patients medical indications (medication intake, diet, physical activity, etc.) 10. Examples of medical information for patients in the portal or questionnaries or programs for life styles improvements (eg. tobacco cessation) b) Call center & medical street support process 1. Tickets (alerts, outbound calls, inbound calls, etc.) manage: we want to know whether we can manage a unique tray of tickets and rules to distribute among operators or one portfolio of patients with its tickets related to one (or more) call center operator/s 2. Example of periodical follow up calls: automatically scheduled in a day/hour; criterion to assign to each operator 3. Vital signs alerts: automatically generating a ticket to be managed; functionalities inside the solution to manage the alert (discard, call, etc.). Alerts originated from a non-loading data after a period or from an out of range value 4. Vital sign alerts: after the treatment by a call center operator, possible referral to a nurse/doctor that is in the street and have to go to the patient's house 5. Alerts: examples of rules creation for a patient or group of patients (ranges, periods, events, etc.) 6. Integration between the Telephonic Platform (PBX) and your solution. Use case: in a follow up call, we want the operator to see the evolution of a patient in the last 3 months (vital signs, medication intake, etc.) 8.
