Community Team?
The Community Team serves as the gateway to Wraparound Services at Saginaw County Community Mental Health Authority. Because the Wraparound processes in based on interagency collaboration, membership on the Community Team may include representation from: Parents or Consumers with Service Experience Education Community Mental Health Department of Human Services Department of Public Health Juvenile Justice Care Coordinators Family Advocates Faith-Based Law Enforcement Community Leaders

What is the

Community Team
1. Targeting and Setting Priorities The Community Team determines which population(s) of families, children and adolescents will receive priority for Wraparound service, taking into consideration the existence of Wraparound contracts, availability of resources, and the needs of various stakeholders. 2. Gate-Keeping/Intake The Community Team is responsible for reviewing referrals of families for Wraparound, presented by the Family Guide, and for making decisions to approve Wraparound Services or recommend alternative supports. 3. Committing Resources / Eligibility Criteria The Community Team reviews evidence from the referral source that the consumer meets Medicaid eligibility for in home services. Eligibility criteria includes: Youth is between the ages of six and 17 Resident of Saginaw County CAFAS scores of 90 under age 12, or a score of 120 between the ages of 13 and 17 Severe complex needs that occur across multiple environments Youth must be receiving Community Mental Health services

Functions of the

Wraparound?
The Wraparound process is a strengths-based system of care designed to improve support delivery for children and families who have been impacted by severe emotional and behavioral challenges, and to preserve families in their own communities whenever possible. Improvement in services means that families and youth receive preferred services for specific issue(s) that occur for time limited periods. These services can be traditional, innovative, creative, and tailored to the individualized needs of a specific family and youth. Wraparound provides coordinated and collaborative supports to families and children, thereby increasing the ability of families to stay together.

4. Wraparound Plan of Care and Family Budget Review The Community Team reviews the plan of care that has been developed by the youth, family, and Wraparound Facilitator. The team reviews for the Wraparound Teams effort to develop a creative, needs driven, preferred service oriented, outcome informed plan. Funding costs for services and support plans may also be reviewed quarterly. 5. Performance Monitoring The Community Team will review outcome data and utilize that information for informed decision making regarding service delivery and support options available to the youth and their families.
