Some of the articles listed on the claim form are neither lost nor damaged, in regards
to those items please call the office and ask to speak to a member of management as the
claim forms intended use is for the loss or damage to items in the shipment.
Unfortunately we are not able to award you the full amount claimed due to the following
reasons:

• When a customer ships his/her belongings without declaring a value to the shipment, the
shipment then travels under a standard liability. This liability covers your goods under ’60
cents per pound per article’ coverage. This means that any item(s) damaged/lost during the
transportation of the shipment without a declared value, are insured only under the ‘60 cents
per pound per article’ liability
** Confirmation of this term and condition can be found on your Moving Contract.

Therefore, the legal liability, which covers your goods during transportation, will only cover
the damages and the losses at 60 cent per pound per article. The total weight of your claim
is 1085Lbs.

$651.00 - $300.00 *Deductible = $351.00

As part of our Customer Appreciation Program, Capital Moving Services would also like to offer you
additional $650.00 towards the resolution of your claim.

Total refund: $1001.00

(Deductible is applicable to every claim processed, all terms and conditions regarding our deductibles could be
found on the front and back pages of the Moving Contract’.)
 Settlement Agreement:

In order for Capital Moving Services to pay you this refund, you will need to agree with the settlement offered.
Your signature is required in order to confirm that you agree with the purposed settlement amount. As soon as
you sign below, please send these documents back to us at the above-mentioned address or fax it to 416-289-
1425 and Capital Moving Services Inc. will send you a cheque for the full amount.

Agreement:
Upon agreeing to the purposed settlement you agree to forever discharge Capital Moving Services Inc from
any further responsibility to any damages or losses involved in this shipment. It is further agreed that the
shipper discharges the carrier from any further claim under this contract, upon the payment of the full-
proposed amount.
