1.0 INTRODUCTION This NSN Accident and Investigation Reporting Policy sets out the principles that all NSN countries should adopt in relation to investigating and reporting all kinds of accidents that occur within Network Implementation, involving Health and Safety. It is designed to enable NSN and its employees to meet the regulatory obligations and NSN Global Policy. NSN shall investigate all work-related accidents, involving, injuries, illnesses or damages to property that may occur inside or outside company premises, including road traffic events on the course of working activities. This procedure applies NSN employees, Suppliers employees or members of the public who may be affected by NSN activities. This policy applies globally within NSN and to its employees and others working on or visiting NSN sites. However, it is subject to any local laws and regulations related to accident reporting. Countries have the discretion to require more restrictive performance than what is in this policy. In the absence of regulatory specific guidance, NSN requirements should be followed. This document will be reviewed when necessary, to accommodate new business activities, best practices and/or regulations changes.
2.0 OBJECTIVE The purpose of this policy is to ensure that NSN employees and Suppliers employees working for NSN immediately communicate the required work-related incidents. This will enable NSN and its Suppliers to comply with all local laws related to reporting to Government Agencies and Offices, as well as provide the means to avoid future occurrences.
3.0 DEFINITIONS Accident Any unplanned event which results in fatality, injury or illness Fatality Death caused by work-related injury or illness Injury Physical harm or damage to a person occurred during work-related activities Lost Days Number of full calendar days a person is off work due to work-related injury or illness. The day injury happened and the day worker returned to work will not be counted. All days in between are counted, including weekends and holidays. It is also included any other full day off due to the same injury or illness, once the employee has returned to work.

Near Miss Any event which had the potential to cause injury, and/or damage and/or loss, but which was avoided by circumstances Illness Sickness suffered by a worker, caused by factors present in the work environment or during activities performed on the job. Work-related illnesses shall be determined by Medical opinion Evacuation The act of removing all workers from a facility / company premise due to any emergency scenario Major Injuries The following cases are defined as Major Injuries: Fatality Amputation Fracture or dislocation of any member Transporting to a hospital in life-threatening condition, work or non workrelated cause Admission to a hospital for overnight stay Loss of sight (temporary or permanent); Chemical or hot metal burn to the eye or any penetrating injury to the eye; Loss of consciousness Any event requiring resuscitation Acute illness requiring medical treatment where there is reason to believe that this resulted from exposure to infected material, chemical or biological agent.

5.0 REPORTING AND INVESTIGATION GUIDELINES In order to properly manage work-related health, safety accidents, each country where NSN operates shall ensure there is an easy and known communication channel, available to all workers report any accident. The country level procedure will ensure that all NSN employees, Suppliers employees and visitors understand their responsibility to immediately report to NSN all work-related accidents. All local regulatory reporting requirements are to be accomplished within the timeframe defined by law. Country level procedure should assign specific responsibility for: Recording accidents. Investigating accidents Implementing corrective actions. Reporting the incident to the enforcement authorities as required. Reporting the incident to the customer as required. Reporting the incident to company insurers as required. Convening an internal board of inquiry for major incidents to be chaired by a member of the country NI senior management team. Conducting accident trend analysis and reporting. Major Injuries reporting All major injuries cases shall be immediately reported to the Global OHS Head, Regional OHS Head and Country NI Head All fatality, amputation and admission to hospital cases shall be immediately reported to the Country Director, Regional OHS Head, Region HR Head and Region NI Head

6.0 RECORDS All work related health and safety accident investigations must be suitably recorded. Each country is required to maintain record of all accidents on electronic and paper records. The NSN Accident Reporting and Investigation Form shall be completed for all accidents. The investigation report should include supporting material where it may help understand the case, such as:
Witness statements. Risk assessments and method statements. Training and other relevant personnel records. Photographs of the general scene of the accident. Photographs should be marked with the date they were taken, the detail of the location where the injury or damage occurred and the name of the photographer.
NSN requires that each Country fully comply with all local reporting requirements established by law
Ensure a Country level Accident Reporting and Investigation procedure is implemented and executed Full implementation of all requirements included in this policy, within the Country Guarantee resources are available to address corrective actions Ensure this policy is understood and, if necessary, completed to cover all local regulatory requirements Promote an open accident reporting environment Full implementation of all requirements included in this policy, in all projects for which is responsible Monitor performance and compliance to this policy, through site audits and inspections Guarantee that all steps within the Country level procedure are executed and followed Ensure all accidents in projects for which is responsible are openly reported and investigated Follow all requirements included in this policy, for all activities to be executed Immediately report all accidents, in accordance to all local regulations and NSN Incident Reporting Policy
