government groups and businesses addressing older adult issues and mature
populations. Specialize in strategic planning and project training and facilitation,

management, and preparation of final report of legislatively mandated AB
1629 Workgroup, responsible for developing AB 1629/ratesetting
methodology recommendations (Medi-Cal Skilled Nursing Facility
Reimbursement Rates). California HealthCare Foundation. 11/08-3/09.

• Spreading Palliative Care in Public Hospitals Project Manager: Project


manager for public hospital grantees planning, implementing, or
expanding palliative care services for underserved communities.

• Real Choice Systems Change Federal Grant, Author: Author of Centers


for Medicare and Medicaid Services grant application for the California
Department of Health Care Services and the California Department of

sites adopting the Coleman Care Transitions Intervention, oversaw pilot
sites dedicated to improving patient transitions from one care setting to

• California Health and Human Services Agency Real Choice Systems


Change Transformation Grant (Centers for Medicaid and Medicare) –
California Community Choices: Wrote Transformation of Information
Technology to Support Systems Change Goal (06/06) in the state
application – grant awarded 9/06; led strategic planning requirement for
the grant (1/07 – 7/07); project consultant for the grant’s Access Goal
which focuses on implementation of two Aging and Disability Resource

• Traumatic Brain Injury Services of California: Public Policy and


Education Consultant for statewide traumatic brain injury network –
provided grantwriting, educational, and policy consulting services under a
California Endowment grant to promote awareness of traumatic brain
injury in California. 1/06-11/07.

• RTZ Associates, Inc.: Survey and summary report of California Area


Agencies on Aging (AAAs) information technology systems, 10/06 –
12/06.

• California Commission on Aging: Strategic Planning Facilitator for the


California Post-White House Conference on Aging Conference –
November 29-30, 2006. Facilitated strategic planning session for the
conference and wrote the final conference proceedings, Turning Ideas into
Action.

• Riverside County Office on Aging and the Riverside County Grandparents


Raising Grandchildren Task Force: Strategic Planning Consultant,
conducted and wrote a strategic plan, fund development plan, and
foundation proposal for a One-Stop-Center for Grandparents Raising
Grandchildren. 10/04-5/06:

• CalCareNet Portal Enhancement Project, California Health and Human


Services Agency: Program Manager; assisted with the development of
modifications and enhancements to California’s aging and long-term care
website, CalCareNet. 5/05- 3/06

7/04 – Present Clinical Social Worker, LifeLong Medical Care, Oakland, CA


Part-time clinical social worker in primary medical care clinic in East Oakland,
CA. Provide clinical social work, case management, and support group services
for dependent and older adults and their families.

8/01 – 6/03 Project Manager, Statewide Resources Consultant, Family Caregiver


Alliance, San Francisco
Provided technical, research, and public policy consulting services to the
Department of Mental Health for the California Caregiver Resource Center
System. Prepared statewide annual reports, conducted research analyses based on
system data, served on statewide long-term care coalitions and work groups,
managed the development of a new statewide web-based client record and
services tracking system, revised the uniform assessment tool for caregivers, and
presented at numerous state and national conferences on caregiving issues.
