Subject: Dropped Object Incident Involving Stabilizer Liner on PH-100 Pipe Handler Product: PH-100 Pipe Handler Affected Assemblies: Clamp Cylinder Assembly, PH-100 Pipe Handler (P/N 30125050) Objective: Advise customers of a previous dropped object incident involving a Stabilizer Liner on a Clamp Cylinder Assembly and recommendation to inspect all affected assemblies. Issue: NOV received a recent report in which a Stabilizer Liner installed on the Clamp Cylinder Assembly broke and fell to the rig floor. No injuries occurred as a result of this incident. During drilling the top drive was subjected to heavy vibration and shock, causing the Saver Sub to hit the Stabilizer Liner (P/N 30125012). The lock wire securing the bolts was broken and the liner was found to have fractured into three separate pieces, see Figure 1. Solutions: NOV has investigated and has found no conclusive evidence as to how this failure occurred. This appears to be an isolated incident. It is strongly recommended that the Clamp Cylinder Assembly be inspected to ensure that the lock wire is installed properly and not damaged. NOV strongly recommends that customers perform post-jarring inspection on their top drives prior to, as well as following jarring or hard drilling, as outlined in the Top Drive Service Manual and in Product Bulletin TDS-93-01. Failure to follow the recommendations and/or guidance in NOV Manuals or Product Bulletins may result in death, bodily injury or property damage. Replacement of damaged and worn parts is important to rig safety and operation. Operators should also refer to previous NOV Product Information Bulletins and Safety Alerts at http://www.nov.com/drilling for additional information related to this issue and information regarding safe operation, maintenance, and inspection criteria.
