A. Introduction:
Engagement Kenway Mack Slusarchuk Stewart LLP (KMSS) was engaged by Sheila Weatherill, former CEO and president of the Capital Health Authority, to tabulate, summarize and report on the expense information and documentation of the office of the CEO of the Capital Health Authority for calendar years 2005-2008. KMSS was also engaged to examine each unique payment requisition form to determine if each contained an appropriate sign off. Source of information The expense documentation was compiled and provided to Sheila Weatherill by Alberta Health Services (AHS) in connection with a request under Albertas Freedom of Information and Protection of Privacy Act (FOIP) on expense account details of the former CEO and president of Capital Health for the Edmonton Area, Sheila Weatherill. Categorization and summarization The expense documentation provided is comprised of a large number of individual documents that are not summarized or organized in detail. For clarity and understanding, the expenses were categorized and summarized into one of four categories: 1) Office, 2) General management and administration, 3) Stakeholder relations and foundation activities and 4) Travel outside of the Capital Health Region.

Notice to Reader: On the basis of expense documentation provided to Sheila Weatherill by Alberta Health Services on expense account details of the office of the CEO and President of Capital Health Authority and information provided by Sheila Weatherill, we have compiled the expense summary schedule (2005-2008) and related notes. We have not performed an audit or a review engagement in respect of this schedule and, accordingly, we express no assurance thereon. Readers are cautioned that this schedule and supporting notes may not be appropriate for their purposes.

D. Notes to Expense Summary Schedule (2005-2008):


(Unaudited- See Notice to Reader) 1) Definition of expense categories:
Office General expenses incurred in running the office not specifically related to Weatherill. Expenses include items such as stationary supplies and office equipment, office catering, staff support, professional development, conferences and periodic subscriptions. General management and administration Day-to-day expenses incurred by the CEO in carrying out management and administrative responsibilities including travel within the Capital Health Region, meeting costs, telephone, organizational memberships, parking, working meals. Also included are recruiting costs for some senior physicians and professional staff, covering airfare, accommodation, hosting (e.g. attending Edmonton events such as Oiler games or arts performances with potential candidates). Stakeholder relations and foundation activities Expenses incurred developing and enhancing relationships with external organizations, primarily other health regions, universities and other post-secondary institutions, municipal governments served by Capital Health and foundations raising funds for Capital Health. Costs include event expenses, catering, and also includes certain one-time costs, primarily associated with the naming and fund raising for the multi-million dollar Mazankowski Alberta Heart Institute. Travel outside the Capital Health Region Travel expenses incurred by the CEO and staff for travel, accommodation and meals outside the Capital Health region including travel to health regions in Alberta, meetings with vendors, with other provincial health and government officials, health research agencies, professional associations and federal government officials.

2) Non Capital Health Expenses Reimbursed:


(a) Certain expenses included within the category of Travel Outside the Capital Health Region, were identified by Weatherill as items that were eligible to be reimbursed by third parties, such as federal committees. No reimbursements were deducted within the Expense Summary Schedule, as reimbursement information was not included in the documents provided to Weatherill by AHS. Reimbursable expenses included within the expense summary are as follows:

(b) Credit card statements included some items of a personal nature. It was found these items were identified, reconciled and fully reimbursed on a regular bases. Page 4

E. Specified Procedure Engagement Report:


Notice to Reader: As specifically agreed, during the compilation of the expense summary schedule covering 2005-2008 we applied a specified procedure to the payment requisitions contained within the expense information. Expense Sign-Off: The specific procedure applied was to determine if the payment requisition contained the sign off of the Board Chair or Vice Chair for Capital Health or after May 2008, by Alberta Health Services. As a result of applying the above procedure it was found that of the 46 unique payment requisitions, 45 of that total contained appropriate sign offs. In performing the above specified procedure we have not performed an audit in respect of the requisitions and, accordingly, we express no assurance thereon.
