G 2/01(d) 4e$ ?a1 Code states that a #erson 1ay not
be a #arty to an in,or1a! 1arriage or e$ecute a
dec!aration o, sa1e i, that #erson is #resent!y 1arried
to a #erson .ho is not the other #arty to the in,or1a!

statutory reDuire1ents o, the .ritten dec!aration are set

each county 1ust #ro)ide a #re-#rinted ,or1 that
inc!udes a dec!aration and oath o, the #arties that states
