Dear Community Partner,

Community Cup Sports (CCS) will be hosting the 6
th
Annual Youth Hockey Tournament at Fort William
First Nation on Saturday the 24
th
of May 2014. We will be targeting Aboriginal high school youth from
the Lakehead District and Catholic School Boards to attend and participate in this one day event. In the
past, this event has attracted up to 75 participants.

The goal of Community Cup Sports is to create a forum where Aboriginal Youth, Community Partners and
Regional police officers can get together and work towards a common goal. We believe that creating this
type of atmosphere will break down barriers and challenge stereotypes and biases that participants may
hold. Each group will experience seeing each other in a different light. This may assist the youth in
making positive choices and considering a career in law enforcement or other areas.

Regional Police officers and Community Partners will be teamed up with high school and elementary
students in a fun filled day playing in this one day Mini-Hockey tournament. Community Partners are
encouraged to attend, participate or set up an information display, listing job opportunities or general
information about their organizations. Parents are encouraged to come out and cheer on their kids.

New this year there will be 2 divisions made up of four teams. Students in grades 6-8 will be one division
and students in grades 9-11 will make up the other division. The tournament is co-ed and each team will
be made up ahead of time based on caliber and grade. Each team will play three games which includes A
and B title games.

The CCS committee will provide lunch for the players and there is no cost to play. Each player must have
their own equipment and players under 18 must wear certified gear and full cages or full masks. There
will be no body contact.

Each player mush also have a consent form signed by a parent or guardian (see below) which can
be faxed back to (807) 623-7730 Attention Bobby Narcisse or emailed to the address listed above.

COMMUNITY CUP SPORTS Youth Tournaments

CONSENT, WAIVER AND ASSUMPTION OF RISKS

The Community Cup Sports Youth Tournaments (to be known as the Activity) occurring at various
sites in the City of Thunder Bay IS NOT MANDATORY on the Participants behalf to meet any work
or educational requirements. The Participant freely accepts and fully assumes all such risks,
dangers, and hazards and the possibility of personal and bodily injury, death, property damage, or
loss resulting from such risks, dangers and hazards. The Participant acknowledges and further
accepts the responsibility of discussing their participation in the activity with their physician and
obtaining adequate medical, health, dental, travel and all other forms of insurance that may apply. By
signing this document the Participant or his/her parent/guardian:

Agrees to freely accept such risks, dangers and hazards inherent in undertaking the Activity.
Understands that no renumeration, compensation, employee benefit, or any other privilege is
entitled as a result of participating in the Activity.
Understands that volunteers are not covered by the Workplace Safety and Insurance Board for
injuries arising as a result of volunteer work.
Agrees that it is the responsibility of the participant to familiarize themselves with environment and
health and safety requirements applicable to the Activity.
Understands and fully accepts that if the participant chooses to participate in any other activity that
is not part of the planned Activity, that they are fully responsible for the consequences of their
conduct.
Understands and fully accepts that if the Participant fails to observe any conditions or rules
established during the course of the Activity that the Participant may be asked to leave.
I hearby grant the Thunder Bay Police Service and those acting on its authorized behalf the right
to:
Photograph or record my person, voice and/or property on digital media, film, video tape, audio
tape or other format for commercial promotion, editorial content, display purposes, or use including
public broadcast.
All rights therein and thereto shall be the exclusive property of the Thunder Bay Police Service to
display and distribute in any manner.
I understand that no additional payments or copies of film, video tape or program elements are to
be provided. In granting these rights, I release the Thunder Bay Police Service from any and all
actions, claims and demands.

I ACCEPT AND FULLY ASSUME all such health and safety risks, dangers and hazards which may
be associated with my participation. Upon the planning committees request I AGREE to leave Arena
should I fail to follow the rules and regulations, or if there is any environmental or health and safety
infraction.
