Duties of the City or Municipal Civil Registrar (C/MCR) or

the Consul General (CG)
Duties of the city/municipal civil registrar (C/MCR) or the consul general (CG):
1) EXAMINE the petition and its supporting documents.
As to completeness of requirements and supporting documents.
Form and contents of the petition:
i. In the form of an affidavit su!scri!ed and s"orn to !efore an#
person authori$ed !# la" to administer oath. %he affidavit shall set
forth facts necessar# to esta!lish the merits of the petition and shall
sho" affirmativel# that the petitioner is competent to testif# to the
matters stated. %he petitioner shall state the particular erroneous
entr# or entries sought to !e corrected or the first name sought to
!e changed and the correction or change to !e made.
ii. %he petition for the correction of clerical or t#pographical error shall
!e supported "ith the follo"ing documents:
1. A certified true machine cop# of the certificate or of the page
of the registr# !oo& containing the entr# or entries sought to
!e corrected or changed'
(. At least t"o )() pu!lic or private documents sho"ing the
correct entr# or entries upon "hich the correction or change
shall !e !ased'
*. Notice or certification of posting'
+. ,ther documents "hich the petitioner or the -.M-/ or the
-0 or 1.-/ ma# consider relevant and necessar# for the
approval of the petition.
iii. In case of change of first name or nic&name the petition shall !e
supported "ith the follo"ing documents and shall compl# "ith the
follo"ing requirements:
1. 1ocuments required in a petition for the correction of clerical
or t#pographical error'
(. A clearance or a certification that the o"ner of the document
has no pending administrative civil or criminal case or no
criminal record "hich shall !e o!tained from the follo"ing:

*. Affidavit of pu!lication from the pu!lisher and a cop# of the
ne"spaper clipping.
iv. %he -.M-/ -0 or 1.-/ shall not accept a petition unless all
requirements and supporting documents are complied "ith !# the
petitioner.
v. %he petition and its supporting documents shall !e filed in three )*)
copies and upon acceptance shall !e distri!uted as follo"s:
1. First cop# to the concerned -.M-/ -0 or 1.-/
(. 4econd cop# to the ,-/0 and
*. %hird cop# to the petitioner.

the petition forms part of the civil register of his office. If it is part of the civil
register of his office he shall assume 5urisdiction.
