A. Required Documents
Form 138 (High School Card) Form 137 (HS Permanent Record) Certificate of Good Moral Character NCAE Result NSO Birth Certificate Chest X-ray Affidavit that you did not attend College in any school yet. (if you graduated in HS before 2014) Reminders: All documents should be photocopied and to be submitted in two sets. Place the original documents in a long brown envelope and the photocopies in another long brown envelope. Write your Surname, First Name Middle Name at the top left part of the envelope using black pentel pen in capital letters (uppercase) ex. CRUZ, MARY JOY ARIOLA

D. Enrollment Procedure (Be sure to let the attending staff /officer to sign in your Route Slip) Step 1 - Submit your Chest X-ray result and undergo Medical Check-up at the Medical Services Department.
Step 2 - Be interviewed by the Head of Academic Programs or Area Coordinator
Step 3 - Submit two sets of the following documents to the Admission Office: Form 137(HS permanent Record) Form 138 (HS Card) Cert. of Good Moral Character NSO Birth Certificate NCAE Affidavit that you did not attend College in case you graduated before 2014.
Step 4 - Let your name be tagged in the Master List on the course you enrolled.
Step 5 - Pay the corresponding fees at the Cashiers Office
Step 6 - Pay other fees collected by the following organizations: Supreme Student Council Obelisk (school paper) Course Organization such as JPIA, HRSS, COUNTS, and CS (Note: BSCOE and BSIE do not have course organization yet, hence, no Payment shall be made)
Step 7 - Request for printing of your Registration Cards (RC) at the Registrars Office
