Consent to possible participation in a commercial event for Taco Bell.

This is to confirm my interest and agreement to be featured in an event/video production for Taco
Bell Canada should I be selected for participation.
If I am selected for appearance in said event/video advertisement for Taco Bell, I agree to the
following and will potentially be compensated as outlined.
Taco Bell Canada, Yum! Brands, Inc. agrees that the use of my image in video footage will not be
used beyond the stated use outlined without further consent.

the video. If you are included in the final edit of the video to be used for advertising
purposes, you will be offered financial renumeration. 5greement to terms of use outlined
