MIST Nationals Transportation Waiver

For all competitors and guests under the age of 18

On 02/08/2014, the participants from MIST Toronto will be departing from _________________
________________________________ to go to College Park, Maryland for the MIST Nationals
2014 trip. This trip will be made via a {rented, charter bus, rented vans, etc}. We ask that all
MIST Competitors and all Guests 18 years of age and under attending Nationals sign and date
this waiver form with their parents to give us permission to transport students to and from the
Maryland-DC area, and transport between the hotel and the Nationals venue.

I, ____________________________________________________, intending to be legally bound
hereby, agree and do hereby release from liability and to indemnify and hold harmless MIST
and any of its agents. This release is for any and all liability for personal or psychological
injuries and property losses or damage occasioned by, or in connection with any activity or
accommodations for this event. Furthermore, the guardian undersigned below assumes all
responsibility for damages caused by their child. The undersigned agree to abide by all the
rules and regulations promulgated by MIST throughout the Tournament Weekend.

This documented agreement constitutes the entire agreement between MIST and
___________________________________________. No waiver, consent, modification, or any
change of terms contained within this agreement shall bind either party unless done so in
writing and signed by both parties. Such a waiver, consent, modification or change, if made,
shall be effective only in the specific instance and for the specific purpose given. There are no
understandings, agreements, or representations, oral or written, not specified herein regarding

We ask that all MIST Coaches and Guests (over the age of 18) attending Nationals sign and
date this waiver form to give us permission to transport the undersigned to and from the
Maryland-DC area and between the hotel and the Nationals venue.
