Information for vehicles and drivers: ALL vehicles must have proof of insurance and
a valid drivers license. Please provide a copy of the Insurance card with this entry
form.

Entry Deadline for line up information is J uly 21
th
5.00 pm. All entries received by the
dead line will receive an email with their line up location by noon Thursday. All late
entries will be put at the end of the parade.

Rules:
- Vehicles must provide proof of insurance (cars, trucks, tractors, etc) with the
entry form
- We ask that participants not throw anything from the unit.
- To maintain a continuous flow we ask that any performances to be displayed be
kept to less than 2 minutes.
- NO ACHOLIC BEVERAGES

Great River Days, Inc. \ City of Muscatine is not responsible for any
injury, damage, loss, theft or vandalism to parade participant and any
others with said organization, its property and employees, caused by any
act, event or occurrence or act of God or other participator, and Great
River Days, Inc. and City of Muscatine shall not be responsible for the
safety and security of your property either during or after the parade
hours, nor shall Great River Days, Inc. \ City of Muscatine be liable to
the parade participant or their employees for the loss or injury from any
such cause. Parade Participant hereby assumes full responsibility
therefore and shall hold Great River Days, Inc. and the City of Muscatine
harmless from any and all loss or damage arising on account of this
agreement.
I accept all rules and regulations attached including assumption of
liability.
